These accidents all appeared to be caused by driver inattention. Following the Kent Police Department Accident Matrix and progressive discipline you are receiving a written reprimand for your three accidents over the last 9 months. I am also recommending that remedial training be conducted by our EVOC team. As a reminder, our department looks at all accidents that officers are involved in over the last two years of service. If you are involved in another preventable accident (with these 3 accidents still on your record) over the next 15 months it will most likely result in time off without pay. On November 27th, 2014 you had responded to a Valley SWAT callout in Federal Way. You were responsible for bringing the Bearcat armored vehicle to the incident. At the conclusion of the incident you returned the Bearcat to the secure impound yard. You began to unload the Bearcat and in the process you set your assigned SWAT rifle on the trunk or roof of your patrol vehicle. A few moments later you were dispatched to a call on 100th Ave SE. You forgot about the placement of your rifle and responded to the call. You had not cleared the call when you realized that your rifle was missing. You notified Sergeant Gagner and immediately initiated a search for the rifle. You retraced your route from the impound lot and located the rifle at the intersection of 1001h and SE 240th St. While the changing out of equipment and after the SWAT callout from the Bearcat to your patrol vehicle has been performed many times, it is unacceptable to have forgotten to secure your rifle. As you realized the moment you noticed your rifle was missing, this is a very serious issue that could have been catastrophic. I do thank you for your honesty in dealing with this issues and notifying your Sergeant immediately. Your actions violate the Kent Police Policy Manual, Section 13.1, Letter J, Equipment Care and Maintenance. Based on the above information you are receiving a written reprimand for your actions on this incident.
