This Change Order amends the above-referenced contract; all other provisions of the
contract that are not inconsistent with this Change Order shall remain in effect. For valuable
consideration and by mutual consent of the parties, the project contract is modified as follows:
1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add
additional work or revise existing work as follows:
In addition to work required under the original Agreement and any prior
Amendments, Contractor shall provide all labor, materials, and
equipment necessary to:
Add bid items for additional work identified in revision 2. Work includes
the following: 52LF of 8-inch ductile iron storm pipe, 2 EA anti-slip solid
locking catch basin lids, and 1 LS for 70 LF Zurn trench drain and ADA
compliant grate. Revision work also increased exisitng pay items 4075
by 2 EA and bid item 4080 and 4111 by 1 EA.
2. The contract amount and time for performance provisions of Section II "Time of
Completion," and Section III, "Compensation," are hereby modified as follows:

In accordance with Sections 1-04.4 and 1-04.5 of the Kent and WSDOT Standard
Specifications, and Section VII of the Agreement, the Contractor accepts all requirements of this
Change Order by signing below. Also, pursuant to the above-referenced contract, Contractor
agrees to waive any protest it may have regarding this Change Order and acknowledges and
accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order, including,
without limitation, claims related to contract time, contract acceleration, onsite or home office
overhead, or lost profits. This Change Order, unless otherwise provided, does not relieve the
Contractor from strict compliance with the guarantee and warranty provisions of the original
contract, particularly those pertaining to substantial completion date.
All acts consistent with the authority of the Agreement, previous Change Orders (if any),
and this Change Order, prior to the effective date of this Change Order, are hereby ratified and
affirmed, and the terms of the Agreement, previous Change Orders (if any), and this Change Order
shall be deemed to have applied.
The parties whose names appear below swear under penalty of perjury that they are
authorized to enter into this contract modification, which is binding on the parties of this contract.
3. The Contractor will adjust the amount of its performance bond (if any) for this project
to be consistent with the revised contract sum shown in section 2, above.
IN WITNESS, the parties below have executed this Agreement, which will
become effective on the last date written below.

I. Proposed Change
Add bid items for additional work identified in Revision 2, work includes the following: 52 LF of 8-inch ductile iron storm
pipe, 2 EA anti-slip solid locking catch basin lids, and 1 LS for 70 LF Zurn trench drain and ADA compliant grate.
Revision work also increased existing pay items 4075 by 2 EA and bid item 4080 & 4111 by 1 EA.
II. Reason and Background for Change
Revision 2 for the project added extra storm measures to the existing plan set. These changes included the addition of
52 LF of 8-inch ductile iron storm main, 2 EA anti-slip inlet grates, 2 additional concrete inlets (BI 4075) and 70 LF of
trench drain and ADA compliant grates. An area of sidewalk at 253rd and Reith Road that sloped toward private
property was identified and Design made the decision to add a trench drain and 8 inch ductile iron storm pipe as
preventitive measures to keep run-off from flowing onto private property. Added revision work also includes adding a
Type I CB and round locking lid to address actual conditions discovered in field that varied from original design.

The purpose of this Action Request is to provide a forward price for installing the two anti-slip castings called out
in revision 2 of the Reith Road Roundabout Project.
NWC will furnish the necessary supervision,labor, and equipment necessary to construct and install the ADA
compliant Non-Slip castings as depicted on sheet 52 for a unit price proposal of$1,180 per EA.The forward
pricing is only for the installation of the ADA compliant Non-Slip castings from STA 11+00— 12+00 shown on
the Reith Road Roundabout plans.
Please issue a change order in the amount of$1,180 per EA for the ADA compliant Non-Slip castings so that the
work can be scheduled and completed. In addition,NWC request that one(1)working day be added to the

The purpose of this Action Request is to provide a forward price for installing the 8"storm called out in revision 2
of the Reith Road Roundabout Project.
NWC will furnish the necessary supervision,labor,and equipment necessary to construct and install the 8" Storm
runs depicted on sheet 52 for a unit price proposal of$110 per liner foot.The forward pricing is only for the
installation of the 8"storm from STA 11+00— 12+00 shown on the Reith Road Roundabout plans.The unit price
includes testing,cleaning,TV inspection,and connection to existing structures.Bedding and backfill will be paid
under bid item 4170.Anything not specifically mentioned above is not included in the unit price for installing the
8"storm.
Please issue a change order in the amount of$110 per LF for the 8" Storm so that the work can be scheduled and
completed. In addition,NWC request that two(2)working day be added to the contract to complete this change in

The purpose of this Action Request is to provide a forward price for installing a HDPE Zurn Trench Drain with an
ADA compliant grate in accordance with the revision 2 Reith Road Plans.
NWC will furnish the necessary supervision,labor, and equipment necessary to construct and install 70 LF of
Zurn HDPE Trench Drain including an ADA compliant Grate as depicted on sheet 52 for a lump sum proposal of
$18,000.The forward pricing is only for the installation to finish grade of the Zurn HDPE Trench Drain with an
ADA compliant Grate from STA 11+00— 12+00 as shown on the Reith Road Roundabout plans.Furthermore,it
is NWC's understanding that any additional items that are not directly related to this work will be paid for at the
already established unit pricing for said items. For example, if any flagging is need for the install of the trench
drain,it will be paid for under bid item 5005.
Please issue a change order in the amount of$18,000 for the Zurn HDPE Trench Drain including an ADA
compliant Grate so that the work can be scheduled and completed. In addition,NWC request that one(1)working
