Dear Mike Carrington:
Cellco Partnership, doing business as Verizon Wireless, values our partnership with the City of Kent (Customer) and
thanks you for your continued business. In support of your communication needs,we are pleased to extend a one-time
equipment offer designed to partially defray deployment costs associated with new and upgrade eligible Smartphone
customers. The terms of this offer detailed below must be accepted and completed by May 31,2024:
Device Discount: Offer is contingent on Customer newly activated and upgrade at the time of
eligibility a minimum of one-hundred-sixty-four(164)Smartphone Government Subscriber Lines on
the "Eligible Device" contained in this agreement. The Eligible lines must be activated on a
Smartphone price plan with a monthly access fee of$39.99 or higher.
"Eligible Device":
• Apple iPhone 14 5G 128GB (any color) at a cost of$0.00,while supplies last.
• Apple iPhone 15 128GB(any color)at a cost of$99.99, while supplies last.
• Apple iPhone 15 Pro Max 256GB(any color)at a cost of$499.99, while supplies last
This device offer is valid through May 31, 2024 only. This offer can be combined with any other promotional
equipment offer currently available. This offer is not available for entities eligible for E-Rate funding. Equipment
discount incentives extended to the City of Kent expressly exclude business from E-Rate eligible customers.
This offer is being extended to the City of Kent (Profile ID 1864471) under the GSA Multiple Award Schedule (MAS)
47QTCA20DOOB5 contract. Please indicate acceptance of this offer by signing and returning the acknowledgement at
the bottom of this letter. Upon receipt of the signed acknowledgement, we will proceed with the application of the
