CONTRACT NAME & PROJECT NUMBER: Lake Fenwick Alum Treatment ORIGINAL AGREEMENT DATE: 9/13/2023 This Amendment is made between the City and the above-referenced Consultant or Vendor and amends the original Agreement and all prior Amendments. All other provisions of the original Agreement or prior Amendments not inconsistent with this Amendment shall remain in full force and effect. For valuable consideration and by mutual consent of the parties, Consultant or Vendor's work is modified as follows: 1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add additional work or revise existing work as follows: In addition to work required under the original Agreement and any prior Amendments, the Consultant or Vendor shall: The Consultant shall provide additional monitoring support for the Lake Fenwick Project. The purpose of the site visit is to refine additional tasks, if necessary, to include in a revised Lake Fenwick Hypolimnetic Aerator Air Line and Davit System Retrofit scope of work. Tetra Tech sent the latest scope of work to the City in December 2023 and following a Teams meeting on January 31, 2024, it was decided a site visit would be helpful in refining the scope. Tetra Tech's senior mechanical engineer will take field notes and photographs as necessary and assist the Tetra Tech PM with revisions to the scope of work based on discussions during the site visit with City staff. Assumptions: • The site visit will take place on a mutually agreed upon date and will be no more than 3 hours in length. If the site visit takes longer than 3 hours a contract amendment will be necessary to cover Tetra Tech's additional time. • City of Kent project point of contact will coordinate with Tetra Tech's PM and senior mechanical engineer regarding availability of City staff. Project Costs Estimated detailed labor hours for Tetra Tech personnel to complete the additional task are shown in Table 1. Table 2 summarizes anticipated project costs including labor and ODCs. Table 3 lists current contract rates for Tetra Tech labor categories for this project. Table 1.
