This Change Order amends the above-referenced contract; all other provisions of the

consideration and by mutual consent of the parties, the project contract is modified as follows:
1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add
additional work or revise existing work as follows:

Amendments, Contractor shall provide all labor, materials, and

In accordance with Sections 1-04.4 and 1-04.5 of the Kent and WSDOT Standard
Specifications, and Section VII of the Agreement, the Contractor accepts all requirements of this
Change Order by signing below. Also, pursuant to the above-referenced contract, Contractor
agrees to waive any protest it may have regarding this Change Order and acknowledges and
accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order, including,
without limitation, claims related to contract time, contract acceleration, onsite or home office
overhead, or lost profits. This Change Order, unless otherwise provided, does not relieve the
Contractor from strict compliance with the guarantee and warranty provisions of the original
contract, particularly those pertaining to substantial completion date.
All acts consistent with the authority of the Agreement, previous Change Orders (if any),
and this Change Order, prior to the effective date of this Change Order, are hereby ratified and
affirmed, and the terms of the Agreement, previous Change Orders (if any), and this Change Order
shall be deemed to have applied.
The parties whose names appear below swear under penalty of perjury that they are
authorized to enter into this contract modification, which is binding on the parties of this contract.
3. The Contractor will adjust the amount of its performance bond (if any) for this project
to be consistent with the revised contract sum shown in section 2, above.

I. Proposed Change
Provide and install gravity block wall as directed adjacent the site perimeter fence. Work to be paid 1 LS
in the amount of $31,242.80.
II. Reason and Background for Change
A gravity block wall is required to properly complete site grading. Site area is extremely restricted and
proper sloping of the ground could not be achieved without vertical steps. Once this was discovered and
understood, discussions were had with the Contractor to define overall limits and wall height, and
negotiate a lump sum cost. This addition will elevate fence height which will improving overall security,
it will prevent soil erosion over new sidewalk, as well as provide a more uniform fence installation
improving aesthetics for the neighborhood.

The purpose of this Action Request is provide T.Bailey with a forward price to construct a CMU
block wall per conceptual drawing provided by the City of Kent along the south and southeast
face of the new West Hill Reservoir.
NWC proposes to perform the subgrade prep,placement of a 4-inch compacted CSTC leveling
course,465 square feet of split face straight CMU block wall installation, and backfill with free
draining material as depicted on the attached plan page for a lump sum price of$27,948.00.
Specific conditions and Exclusions are as follows:no engineering,drawings, survey,block
doweling/epoxy,wall drain system, and a maximum block wall height of 36-inches.Wall area in
excess of 465 square feet will be tracked and billed out accordingly.
Please provide NWC with timely response to this Action Request so that we may coordinate any
submittals, and material procurement in a timely manner with the overall project schedule and

It was determined in the field that the West Hill Reservoir site will need a short retaining wall
(N3') added to account for a grade break between hard surfaces and vaults on the southern
portion of the site. The proposed wall length is approximately 150' long and will be
approximately 465 square feet of split face CMU blocks. This general contractor markup, the
contractor's proposal to complete the construction of this added site feature is $31,301.76.
Are you okay with proceeding with a change order that will add this work to our contract?

Please review and comment on the attached proposal from T Bailey's subcontractors price to
install a gravity block wall. I've asked if this included a cap block. They excluded epoxy, caps
need some type of adhesive.
This proposal is for +/- 150 LF of wall. They didn't break out the 85 plus feet that is required
along the sidewalk. That number alone is +/-$16,768.80.
All of these numbers will need the 12% subcontractor markup.

1. Does the City have at least preliminary or conceptual drawing they can provide?Attached
2. Will wall have a drain line? No drain piping, Attempt to place free draining material against the blocks
3. Does the 30" overall height include bury depth and leveling rock course, or is this the exposed face?
The overall height will be 36 inches There will need to b +/-4 inches of compacted CSTC.
4. Does the City have particular block style/face in mind? Split face straight

The City of Kent would like a block wall priced out. It was my understanding Paddy was on site last week
and NW Cascade would be interested in building.
Wall will be located on the south side of Reservoir and extend from the flow control structure past CB
13.The wall will have a straight run length of 85' (just north of new sidewalk) with both ends having a
dog leg. The leg to the west will be 10' and the leg to the east 55'. Each end will taper down. Height of
wall 30"to include a cap block. Color would be gray.
If interested could you get us a price as soon as possible please.Think this would be on the front end of
the site grading activities.

Please review, comment and or approve the proposed change order for a retaining wall along the
south and east portion of the Reservoir. It will create a level level for the fence. It will create and
more aesthetically pleasing and possibly secure site.

A few questions regarding wall:
1. Does the City have at least preliminary or conceptual drawing they can provide. Attached
2. Will wall have a drain line? No drain piping, Attempt to place free draining material against the blocks
3. Does the 30" overall height include bury depth and leveling rock course, or is this the exposed face?
The overall height will be 36 inches There will need to b +/-4 inches of compacted CSTC.
4. Does the City have particular block style/face in mind?Split face straight
5. Does the City want the blocks epoxy dowelled together? Dowels will not required

The City of Kent would like a block wall priced out. It was my understanding Paddy was on site last week
and NW Cascade would be interested in building.
Wall will be located on the south side of Reservoir and extend from the flow control structure past CB
13. The wall will have a straight run length of 85' (just north of new sidewalk) with both ends having a
dog leg.The leg to the west will be 10' and the leg to the east 55'. Each end will taper down. Height of
wall 30" to include a cap block. Color would be gray.
If interested could you get us a price as soon as possible please.Think this would be on the front end of
the site grading activities.
