PURPOSE This Addendum to LIHWAP Vendor Agreement is entered in order to provide the Credit Benefit for the existing LIHWAP benefit recipients of the program year starting 10/1/2022 and ending 9/30/2023. We, the Agency and the Vendor, agree to the aforementioned additions to the LIHWAP Vendor Agreement agreed and signed in 2021 for the contract period between 10/1/2021 and 9/30/2023. AGENCY RESPONSIBILITIES 1. The Agency is adding Credit Benefit towards LIHWAP recipients. The existing LIHWAP recipients will be eligible to receive a credit of$500 on each of their water and sewer account(s), not greater than$1000 in total as long as their accounts are valid at the time of providing this Credit Benefit. This Credit Benefit will be only sent directly to LIHWAP vendors. 2. The Agency will start the Credit Benefit process after the customer's balance is $0.00 meaning the Agency will do business as usual,requesting customer's owing balance and pledging the owing balance first, then the Credit Benefit process will come shortly afterwards. VENDOR RESPONSIBILITIES 1. The LIHWAP recipients (customers) cannot request remaining credit balances from LIHWAP vendors if customers move or close their account(s). 2. If the customer's account is closed and this closed account still carries a credit balance from the Credit Benefit, this credit balance should be communicated with and refunded immediately to the Agency with a list of customers' names, account numbers, and addresses before or after the LIHWAP contract end date 9/30/2023. 3. Termination This agreement may be terminated by either party with a thirty (30) day written notice to the other party. Termination shall not extinguish authorized obligations incurred during the term of the agreement. If funding is withdrawn, reduced, or eliminated by Commerce, the agency has the right to terminate this agreement immediately.
