March 18. 2021 This Change Order amends the above -referenced contract; all other provisions of the contract that are not inconsistent with this Change Order shall remain in effect. For valuable consideration and by mutual consent of the parties, the project contract is modified as follows: 1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add additional work or revise existing work as follows: In addition to work required under the original Agreement and any prior Amendments, Contractor shall provide all labor, materials, and equipment necessary to: Added type 8 junction box to accommodate new power pole installation. This CO includes all material, equipment, tools and labor necessary to supply and install the new handhole. 2. Reason and Background for Change Original contract for project 18-3003 did not account for necessary handhole installation to accommodate the installation of the new PSE power pole. Field discussion with PSE determined that an additional handhole was necessary to reset the degrees of bend in the conduit run from the compressor building to the power pole. The handhole is used to connect previously existing conduit to the new conduit that makes a 90 degree bend toward the new power pole location. III. As used herein, a significant cost increase shall mean any increase in cost of materials exceeding 6% experienced by seller from the date of the contract signing. 17) Delivery pricing is based on Oldcastle Infrastructure's current fuel surcharge rate and is good for 7 days from the date stated on this Quote. Thereafter, Customer is responsible for paying Oldcastle Infrastructure's fuel surcharge rate in effect on each delivery date. Fuel surcharge rates are derived from pricing, as established by the U.S. Energy Information Administration's Gasoline and Diesel Fuel Index, published at httos://www.eia.aov/petroleum/aasdiesel/ Additional Items: 18) Every effort has been made to provide an accurate take -off however, the quantities are not guaranteed by Oldcastle but provided for the buyer's convenience only. Please let me know if I have missed anything. (I will continue to follow up with status on the new float drawing) TRENCHING/ELECTRICAL: 1) Tin at PSE to provide date of new pole installation the week of 3-21-22. 2) Brian to advise if the city will provide the handhole or if a change order is required. 3) Brian to advise on the backfill for the new handhole area — to ensure people are not able to drive over that area. 4) Once we have the new pole installation date AUS to trench (3'6" deep at minimum) and coordinate with electrical sub to install conduit and handhole. 5) Electrical sub to coordinate L&I Inspection of the conduit and handhole install so AUS can backfill same day. Must be completed a minimum of 3 days prior to PSE installing new pole. 6) AUS to leave a 4'X4' opening near the new pole location per PSE. AUS will provide plywood, caution tape and cones to secure the area. Approved by Brian and Jason. 7) Electrical sub to finalize building, panel wiring and call for another L&I inspection for that portion of the work. 8) PSE to activate power. I will send a schedule of these events once we have the date from PSE. RFI: 1) Corky to submit RFI for WDF Variance request with specific in water dates to remove old aeration system and set the new HDPE pipes. Brian to provide the drawings of the old system so we know how many days will be needed, this will determine the dates. 2) Brian to confirm if any of the old aeration system is to be kept by the city. Response: • Trenching and backfill for secondary utility power is not addressed in the PSE Service Handbook. A description based partially on the basic requirements for primary trenching and backfilling would be to use a 36" minimum depth to top of raceway, 3" sand bedding, and native or select backfill with stones greater than 4" removed and machine compacted in 6" lifts. Provide tracible warning tape 12" below grade. Contractor is responsible for providing secondary conduit and wire between the Compressor Building and the new utility pole. The riser at the utility pole shall extend 10' above grade and conductors shall be long enough to extend above that height to weatherhead. All work above 10' at pole will be performed by PSE using Contractor -provided materials.
