This Change Order amends the above -referenced contract; all other provisions of the
contract that are not inconsistent with this Change Order shall remain in effect. For valuable
consideration and by mutual consent of the parties, the project contract is modified as follows:
1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add
additional work or revise existing work as follows:
In addition to work required under the original Agreement and any prior
Amendments, Contractor shall provide all labor, materials, and
equipment necessary to:
Added type 8 junction box to accommodate new power pole installation.
This CO includes all material, equipment, tools and labor necessary to
supply and install the new handhole.
2. The contract amount and time for performance provisions of Section II "Time of
Completion," and Section III, "Compensation," are hereby modified as follows:

In accordance with Sections 1-04.4 and 1-04.5 of the Kent and WSDOT Standard
Specifications, and Section VII of the Agreement, the Contractor accepts all requirements of this
Change Order by signing below. Also, pursuant to the above -referenced contract, Contractor
agrees to waive any protest it may have regarding this Change Order and acknowledges and
accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order, including,
without limitation, claims related to contract time, contract acceleration, onsite or home office
overhead, or lost profits. This Change Order, unless otherwise provided, does not relieve the
Contractor from strict compliance with the guarantee and warranty provisions of the original
contract, particularly those pertaining to substantial completion date.
All acts consistent with the authority of the Agreement, previous Change Orders (if any),
and this Change Order, prior to the effective date of this Change Order, are hereby ratified and
affirmed, and the terms of the Agreement, previous Change Orders (if any), and this Change Order
shall be deemed to have applied.
The parties whose names appear below swear under penalty of perjury that they are
authorized to enter into this contract modification, which is binding on the parties of this contract.
3. The Contractor will adjust the amount of its performance bond (if any) for this project
to be consistent with the revised contract sum shown in section 2, above.

become effective on the last date written below.

I. Proposed Change
Added type 8 junction box to accommodate new power pole installation. Payment includes all material,
equipment, tools, and labor necessary to supply and install the new handhole. Cost total $10,129.94 as
shown on FA sheet 001.
II. Reason and Background for Change
Original contract for project 18-3003 did not account for necessary handhole installation to
accommodate the installation of the new PSE power pole. Field discussion with PSE determined that an
additional handhole was necessary to reset the degrees of bend in the conduit run from the compressor
building to the power pole. The handhole is used to connect previously existing conduit to the new
conduit that makes a 90 degree bend toward the new power pole location.

Oldcastle Infrastructure Commercial Clarifications

Special Products:
1) Any special product(s) and high -volume standards (collectively, "Special Products") will be invoiced on a mutually agreed upon in writing
delivery date and ownership transferred, upon Invoicing. Oldcastle Infrastructure, Inc. ("Oldcastle") reserves the right to require full or partial pre-
payment for any Special Products ordered.
2) Special Products may not be returned.
3) If Special Product(s) cannot be accepted within 30 days of invoicing, a 10% storage fee will be assessed monthly unless otherwise agreed
upon in writing.
4) All Special Products ordered as part of a cash sale must be fully paid prior to production.
5) A disposal fee of $200/ton will be charged for all Special Products on hand over 90 days from the mutually agreed upon delivery date.

6) The site must be accessible by delivery vehicles under their own power. If the material is to be delivered and set by Oldcastle, acceptability tc
the site will be determined by the boom operator.
7) Freight charges quoted are based on full truckload quantities. Short loads will be subject to additional charges to cover the cost of delivery.
8) Deliveries canceled with less than 24 hours' notice are subject to full delivery charge.
9) Delivery includes one hour for offloading. Additional time in excess of one hour will be invoiced at an hourly rate.
10) A restocking fee of up to 40% may be charged on undamaged, standard products. Freight charges for returned product on Oldcastle

11) All Returns must be approved by Oldcastle. Returns will not be accepted after 90 days from date of purchase.
Pricing:
12) Oldcastle must be notified if a project requires prevailing wages as additional costs may be applied to the quotation.
13) Pricing is valid for 30 days from the date of this quote.
14) A 2% surcharge will be imposed on all Credit Card transactions, which is not greater than our cost of acceptance. A surcharge will not be
applied to any ACH or Debit Card transaction.
15) Pricing remains valid for 180 days from order confirmation. For orders that have not shipped or transferred ownership to the buyer after 180
days, we reserve the right to implement a 1.5% quarterly price escalation fee.
16) If during the performance of this contract the cost of materials significantly increases through no fault of the seller, we reserve the right to
equitably adjusted the price of this contract by an amount reasonably necessary to cover any such significant increase in the costs of materials.
As used herein, a significant cost increase shall mean any increase in cost of materials exceeding 6% experienced by seller from the date of the
contract signing.

Hi Brian,
I wanted to send a follow up email reiterating our task list from our meeting this morning. Please let me know if I have
missed anything. (I will continue to follow up with status on the new float drawing)
TRENCHING/ELECTRICAL:
1) Tin at PSE to provide date of new pole installation the week of 3-21-22.
2) Brian to advise if the city will provide the handhole or if a change order is required.
3) Brian to advise on the backfill for the new handhole area — to ensure people are not able to drive over that area.
4) Once we have the new pole installation date AUS to trench (3'6" deep at minimum) and coordinate with
electrical sub to install conduit and handhole.
5) Electrical sub to coordinate L&I Inspection of the conduit and handhole install so AUS can backfill same
day. Must be completed a minimum of 3 days prior to PSE installing new pole.
6) AUS to leave a 4'X4' opening near the new pole location per PSE. AUS will provide plywood, caution tape and
cones to secure the area. Approved by Brian and Jason.
7) Electrical sub to finalize building, panel wiring and call for another L&I inspection for that portion of the work.
8) PSE to activate power.
I will send a schedule of these events once we have the date from PSE.
RFI:
1) Corky to submit RFI for WDF Variance request with specific in water dates to remove old aeration system and
set the new HDPE pipes. Brian to provide the drawings of the old system so we know how many days will be
needed, this will determine the dates.
2) Brian to confirm if any of the old aeration system is to be kept by the city.
CHANGE ORDERS:
1) Corky to submit change orders for the following: (once confirmation has been decided)
a. Handhole, labor, and equipment to install
b. Run new power to the water machine outside of the compressor building
Paint/labor— door and louvers (2 front, 1 back)
IN WATER WORK: (if variance is approved)
a. Brian to have sergeant remove the vagrants from the boat launch area on the date we set for in water work
b. AUS to send Dive crew to remove old aeration system (estimating 1 week worth of work, will know more with
the old plans)

d. AUS to drop in the 24" and 72" HDPE pipes in preparation of new aeration float

Hi Brian,
Quick update, locaters will be out on Wednesday 3-16-22. We went to the site and marked the area in white
paint. Once we get cleared to dig the trench will dig and get the conduit/pull string in. I will coordinate with PSE and let
you know the date for the pole installation.
Michael is making headway with the float engineers, and we are hoping to have new drawings finalized, he is waiting on
one more confirmation and will send me the new drawings and any additional comments needed for TT to review.
Reminder that put a new lock on the inside of the roof hatch until you can get a city issued lock or you can keep the lock
we put on. The code is the last 4 digits of your cell number.

Hi Brian,
I just wanted to touch base with you to let you know that Michael is making progress with the Engineering team for the
float. They are hoping to have everything dialed in early next week so I can re -submit the new drawings and answers to
questions TT had on the last submittal.
Once we get a day or two of dry weather, we will paint the exterior of the building.
I have a call into 'call before you dig' and plan on meeting them early next week to ensure our trench plans do not
interfere with any underground wiring. Once I meet with them, we will trench, lay the conduit with a pull string. I will
then coordinate PSE and our Electricians to dispatch on the same day to ensure to the new wire is not stolen.
Per our conversation today, I will submit the RFI for the Fish and Wildlife Variance.
Once I have a better idea of the float fabrication, I will also submit a new schedule, it is difficult to submit the schedule
until we are approved for fabrication.

POWER GENERAL NOTES - COMMERCIAL PROJECT
1. All materials to be installed in accordance with Puget Sound Energy's (PSE) standards. Any deviation from
this work sketch must be AUTHORIZED by PSE's Project Manager and NOTED on the Foramens' Copy.
2. All switching arrangements and/or outage arrangements are to be made with the Project Manager at least
three (3) working days in advance.
3. Contact the Utilities Underground Location Center (1-800-424-5555) at least 48 hours prior to commencing
work to get the underground facilities located.
4. STAKING: The customer will provide all staking (transformer, handhole, trench, grade, lot, pole, sidewalk,
etc.). See sketch and details for locations. Equipment locations must be approved by the Project Manager.
5. SITE PREPARATION: The work area will be at or near finished grade, clear of trench spoils or construction
materials which would restrict construction and/or equipment access, before work can begin.
6. Roads shall be paved or have a compacted , crushed rock base in place.
7. CLEARANCES: Transformers require a minimum of 6 feet from fire fighting equipment, 10 feet from
combustible walls, overhangs, doors, and windows, and a minimum of 5 feet from the back of curb (or guard
posts will be required per PSE standards). All conduits and vaults are to be at least 5 feet away from water,
storm and sewer lines when paralleling them in the fight of way, and at least 1 foot when crossing them, or
as required by Your local municipality.
B. All work is to be done in accordance with local municipal and county permit requirements as applicable.
9. Customer/Developer is responsible to provide, Install and maintain all secondary service cables, conduits
and crossings from the individual units meter base to the designated connection point at top of pole.
10. Inclement weather conditions may cause delays in construction times and dates.
11, EXCAVATION. The customer is to provide all trenching, backfill, vault excavations, compaction and
restoration per this sketch and per PSE standards. A minimum protective cover of 36" is required over
PSE's primary voltage equipment and 24" is required of PSE's secondary voltage equipment. The customer
will provide any and all shoring or they will side slope the trench to 1:1.

Response:
• Trenching and backfill for secondary utility power is not addressed in the PSE
Service Handbook. A description based partially on the basic requirements for
primary trenching and backfilling would be to use a 36" minimum depth to top of
raceway, 3" sand bedding, and native or select backfill with stones greater than
4" removed and machine compacted in 6" lifts. Provide tracible warning tape 12"
below grade. Contractor is responsible for providing secondary conduit and wire
between the Compressor Building and the new utility pole. The riser at the utility
pole shall extend 10' above grade and conductors shall be long enough to extend
above that height to weatherhead. All work above 10' at pole will be performed
by PSE using Contractor -provided materials. Figures 1 & 2 from the PSE Service
Handbook are included below:
