For valuable consideration and by mutual consent of the parties, the project contract is modified as follows: 1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add additional work or revise existing work as follows: In addition to work required under the original Agreement and any prior Amendments, Contractor shall provide all labor, materials, and equipment necessary to: This change order revises the contract by adding scope and payment for installation of the traffic islands in the intersection of S 228t" St and 96tn Ave S. The traffic island will be built in accordance to sheet 38A of Revision 5. The cost of this change order totals $13,172.14 2. Reason and Background for Chanqe Icon Materials constructed the traffic islands found at S 228th St and 96th Ave S in accordance with the Issued For Construction plan set on the 2022 Overlays (sheet 38). After construction, the City of Kent determined that the islands did not provide appropriate turning radii for traffic and Revision 5 was issued. Brian was able to get them to revise pricing for their scope of work, but their subcontractor was unable to provide any reduction in pricing for their curbing/painting/striping work. Are you okay with proceeding with a CO to complete the revision at this intersection? Please let me know if you have any questions or need any additional information.
