consideration and by mutual consent of the parties, the project contract is modified as follows:
1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add
additional work or revise existing work as follows:

In accordance with Sections L-A4.4 and 1-04.5 of the Kent and WSDOT Standard
Specifications, and Section VII of the Agreement, the Contractor accepts all requirements of this
Change Order by signing below. Also, pursuant to the above-referenced contract, Contractor
agrees to waive any protest it may have regarding this Change Order and acknowledges and
accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order, including,
without limitation, claims related to contract time, contract acceleration, onsite or home office
overhead, or lost profits. This Change Order, unless otherwise provided, does not relieve the
Contractor from strict compliance with the guarantee and warranty provisions of the original
contract, particularly those pertaining to substantial completion date.
All acts consistent with the authority of the Agreement, previous Change Orders (if any),
and this Change Order, prior to the effective date of this Change Order, are hereby ratified and
affirmed, and the terms of the Agreement, previous Change Orders (if any), and this Change Order
shall be deemed to have applied.
The parties whose names appear below swear under penalty of perjury that they are
authorized to enter into this contract modification, which is binding on the parties of this contract.
3. The Contractor will adjust the amount of its performance bond (if any) for this project
to be consistent with the revised contract sum shown in section 2, above.

NEW PAY ITEMS I Notapplicabte
ay of 112th Ave SE and SE 235th St, a sink hole was
iscovered in the roadway on SE 23sth St, east of it's intersection with 112th Ave SE. The sinkhole
ppeared to be directly over an existing storm pipe. City of Kent TV inspection of the storm run revealed
that the existing 58"x36" CMP arch pipe had significant failures starting at the manhole located on SE
235th St heading easterly approximately 115 LF. Design and Operations determined it was necessary to
repair this section by replacing the failed arch pipe with 36" diameter ADS triple walled pipe, and install
two new manhole structures; one intersecting two existing lateral storm runs and one to connect the
new ADS pipe to the old arch pipe. Due to the nature of this work, siginificant impacts to the existing
roadway surfacing on SE 23sth St are necessary; a grind and overlay of the area is being performed to
resurface the road, following the completion of the pipe replacement and structure installation.

Per our Teams conversation, here is a list of items Lakeside would like to know the City's
preference on prioritization for:
1) 235th x 112th Storm Repair (added work to contract)

2) I44Eh Traffic Calming (added work to contract)
3) t44rh Road Raising (orig. contract; sh 57 etc)
4) 248th Grind/Overlay (orig. contract; sh 11 etc)
Our contractor is happy to try accommodating our request for priority, but did mention that we
will be running out of paving weather shortly; based on recent years'weather, I agree with this
assertion. This week and early next week, Lakeside will be completing the ADA ramp on 132nd to
stay ahead of WSDOT's paving window (to avoid needing a paving variance d/t the HMA
patching in front of the new curb and gutter). In what order would you prefer they attack the
abovementioned areas, after they complete the ADA ramp on 132nd x Kent-Kangley?

Please see updated pricing for the 235th Pipe repair. We have deleted the pea gravel and reducer nipple then added 40
LF of 12 inch Ductile for the side hook ups. lf we are going to go forward with this we would like to pot hole right away to

In my initial email asking for proposal, I failed to include that the City would like 40' of L2"
Ductile Iron Pipe installed, as well. This will be for the lateral runs into the 60" structure. Please
provide pricing for this.
Omit the pipe nipple from the proposal; the current repair plan does not include this item.
The City requires CSTC (5/8 minus) for pipe bedding; no pea gravel.
Does this proposal include shoring, all removal, hauling, and disposal of waste material (such as
the existing pipe)?

Please provide a change order price proposal for the below described work.
Ed with Accurate was concerned about the structure size, but both the City of Kent detail (5-2)
fortype 2, and the WSDOT (b-10-20.02), shows a table that allows the below mentioned
structure sizes to work. Also, we shouldn't need a reducing nipple for the 36"x58'CMp arch pipe,
as the 84" structure can take up to a 60" metal walled pipe, with no reducing nipple. Please
inform the City if there is a variable that is not being accounted for that is preventing the use of
these structures, per the detail.

Please provide pricing for the below listed items in relation to the 235th Pipe replacement. For
the pipe material, we are interested in pursuing this work in a timely fashion, so pipe type can
be discussed as far as availability, cost, etc.
1) Pothole to verify depth of existing pipe and the watermain to the north of the run;
2) Remove 115'of the damaged 58"x36" arch pipe from the MH at the intersection of 112th &
235th, going East;
3) Replace that pipe with 36" diameter pipe (we are hoping for PPE but are open based on
price and availability of other pipe types, as well);
4) Set a 60" type 2 structure at the nodes of the two 12" diameter runs tying in from
structures Y236 & Y235, and connect these runs to the new structure;
5) Setan84" type2structureattheterminationofthell5'run(endof removal limits)and
connect the existing 58"x36" pipe (from the East) to the new 36" run via this structure.;
6) Overlay full width of 235th to the easternmost extent of this pipe replacement work.
Pipe types we are hoping for pricing and availability on are:
- Corrugated polypropylene (triple walled)
- Ductile iron
- PVC (various SDR, if applicable)
- Concrete

I have discussed this with Operations staff including Jens and Dave and my recommendation is
below, Please review and let me know if you have any questions.
r Pothole to determine depth to storm pipe. Call for locates, might be worth potholing to
find the watermain to the nofth as that will likely be encountered during construction,. Remove 115'of the damaged 58"x36" detention pipe from the MH at the intersection of
112th and SE 235th St going east.. Replace 115'with 36" diameter pipe. Pipe type to be determined based on availability and
cost.
o Note; 36" will decrease the capacity of the pipe but 48" will not provide sufficient
cover.
Set a 60" Type 2 structure at the node of the two L2" diameter CBs (Y236 and Y235) and
connect these runs.
Set an 84"Type 2 structure at the termination of the 115'of pipe to connectto the
existing 58"x36" CMP pipe.

Hey guys,
The pipe looks like it was crushed and there is roadway damage on the google earth screenshot
(attached) as well as a utility marking in the vicinity of where the pipe collapse. We should
pothole to determine actual coverage of the pipe.
The as-built shows 5.65'to IE of current 38" pipe. Putting a 48" pipe would be 1,65'of coverage,
What is the manufacture coverage rating on CPP?

Atthis point, we have only asked Lakeside to start preparing a price proposal. No work has been
directed beyond placing steel sheets over the known roadway failure area to attempt minimizing
a safety hazard. Let me know if there is anything Construction Engineering can do to help out.

Brian,
I am good with the plan but we need to make sure we have cover for the taller diameter pipe. I
checked with Ozzie and he said often times they only used the arched pipe because of issues
with cover, On the bright side, as you said Brian, we will be covering it with crushed rock instead
of the native that was likely used before.

Yes, I think the best course of action at this point is to replace the line in its entirety. As far as
the pipe material is concerned, I think that CPP is perfect for this. The 48" pipe is triple walled
and again, I think it's perfect for this run.

Thanks for getting your crew to help out so quickly today. As Lakeside/their subs will likely be
performing this work under a change order to the existing overlay project, Eric has asked me to
work with them on getting a price estimate.
A couple of questions I'd like to have your input on:
1) Will we be replacing the approximate 250'run between structures (112th to SE 23sth Pl,
traveling longitudinally with 23sth St), in its entirety?
2) What material will be replacing the existing 58" x 36" CMP with? Chris and Drew were
informally discussing 48" poly-pro pipe as an option.
I will go ahead and have Lakeside start working on getting information ready for an estimate,
but these details will help us dial in a more accurate number.
