In accordance with Sections 1-04.4 and 1-04.5 of the Kent and WSDOT Standard
Specifications, and Section VII of the Agreement, the Contractor accepts all requirements of this
Change Order by signing below. Also, pursuant to the above-referenced contract, Contractor
agrees to waive any protest it may have regarding this Change Order and acknowledges and
accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order, including,
without limitation, claims related to contract time, contract acceleration, onsite or home office
overhead, or lost profits. This Change Order, unless otherwise provided, does not relieve the
Contractor from strict compliance with the guarantee and warranty provisions of the original
contract, particularly those pertaining to substantial completion date.
AII acts consistent with the authority of the Agreement, previous Change Orders (if any),
and this Change Order, prior to the effective date of this Change Order, are hereby ratified and
affirmed, and the terms of the Agreement, previous Change Orders (if any), and this Change Order
shall be deemed to have applied.
The parties whose names appear below swear under penalty of perjury that they are
authorized to enter into this contract modification, which is binding on the parties of this contract.
3. The Contractor will adjust the amount of its performance bond (if any) for this project
to be consistent with the revised contract sum shown in section 2, above.

Jason,
NW Cascade is planning to be up there Monday to see about getting down to the valve nut.
We're going to start by using vactors within a L8-24'sleeve dropped down as they vac from top.
Water department is allowing them to use one of their sites for decant off of LL4rh south of Kent-
Kangley on west side. I do know friction on sleeves that deep can be substantial depending on
soil types, stiffer the sleeve material the better, but no guarantee's. Even so, worth a shot. Did
one 18 ft deep down in valley on S 228th to window Verizon fiber for bridge shafts, was
successful but still harder than could have figured going in.
Talked about using a 6" PVC 3035 sewer pipe as new valve stem riser like Lacey does to prevent
all the kinks from setting several 6'valve can riser sections, otherwise we'll have to band two
full length stiffeners to outside of valve cans so they can be dropped down and backfilled plumb.
Also, talked about a proprietary foam plug (forget name) that goes on bottom of riser around
valve nut that works to prevent boiling sand from coming back up within riser.
If getting a sleeve down there works we'll be real money ahead, no dewatering, baker tank,
shoring boxes, significant surface restoration, etc; but at end of day, we'll just have to see how
things go and ratchet up as conditions require.

Our contractor (Northwest Cascade) will begin working to expose and provide access to the
buried water valve as we discussed Friday on our main at the northern limits of the Verdana
(Bridges) development, The cost for this work will be tracked in the field on force account and
paid via change orderon our 2O2L Watermain Replacement project (21-3002)' We anticipate
the final cost will exceed $20,000.

NW Cascade began excavating to access the water valve @ 8am. NW installed a 10'piece of 24"
PVC pipe as a c;sing and began excavating material with a vactor truck. The vactor truck was
having a very difficult time due to large rocks. I had them excavate a little deeper with the track
hoe and we began using the vactor again. We were able to exposed the valve nut stand pipe and
we began cleaning it out. We could get down to the operating nut but every time they pulled the
vacuum pipe out ihe water and dirt rush back in and fill the stand pipe up. We decided to add an
additional 4' of 24" casing. (Sorry I mentioned 18'earlier but I was wrong). We began removing
additional material inside the casing attempting to get down below the operating nut. I believe
this will allow the water to have somewhere to go and not plug up our stand pipe. The second
vactor truck became full @ 330pm. We decided to secure the hole and call it for the day' We will
begin in the morning removing additional material in the casing. I'll keep you in the loop as soon
as I have new information.
Our water department has decided to add a L6" butterfly valve inside the vault' They are
supplying all of the parts but they want NW to install the valve. They had to get one part from
Kentucky so they won't have everything till Thursday. Again I will keep you in the loop as things
progress.

Hi Eric,
Everybody on board with the extra cut-in valve work the water department is requesting?
Definitely makes good sense, but don't want to be violating any approval rules.
If City provides big parts, I'd hope costs would be under $10,000 for miscellaneous parts,
equipment and labor.

Yeah, if two vactor trucks can't keep up, we're looking at a dewatering system. Static water
table was about -7 ft when I checked last Friday and top of valve is approximately -25.
Would say we need to look at sinking a perforated casing a couple feet below the main, dump
some washed rock in bottom and pump into a tank, drain off into swale around building, then try
again. Take some pressure off.
Otherwise, we'd probably need to postpone until September/October, but deep well still likely
based on what I'm seeing and hearing.

Jason,
NW Cascade began @ 7am. Upon arrival NW removed the short 4' piece of casing and added a
10' piece. They pushed the casing down as far as possible until they felt like it was touching the
top of the watermain. NW began excavating with the vactor truck @ 8am. @ 9am the vactor
truck broke down. They got it repaired @ 11am. NW continued excavating and was able to get
below the operating nut. During the excavation NW discovered the stand pipe was broken in a
few spots, They removed the broken pieces and began attempting to install the valve nut riser
extension, They were not successful in installing the extension. The ground water is coming in
two fast. Even when using two vactor trucks they can not keep up. NW began securing the site
@ 430pm. We dumped the second vactor truck at the decant station and called it a day.
I think it's time to step back and come up with an actual design. With the amount of water and
depth of the pipe, we should give this some real thought about how to move forward.
They will not be on site tomorrow, but we have vactor trucks on order for Thursday if we decide
to proceed.

Good Morning Again,
I just talked with Jason and Jason about this. Consensus, and direction, is to not burn any more
money until we have a different plan in place. We are concerned the valve could be broken, but
there is no way to tell without getting someone down there. Getting a person down that deep
would require dewatering, engineered shoring, and a much larger disturbance area. We can
keep trying, but I believe if it could be done with the methods we are trying it would have been
successful by now. Keep in mind that we need to get there once to close the valve, then one
more time to open it back up after the new valve is installed in the vault.
I know you three are all very busy, but please respond and let me know when we can schedule a
time to discuss a plan for going forward,

Good Morning Again,
I just talked with Jason and Jason about this, Consensus, and direction, is to not burn any more
money until we have a different plan in place. We are concerned the valve could be broken, but
there is no way to tell without getting someone down there, Getting a person down that deep
would require dewatering, engineered shoring, and a much larger disturbance area. We can
keep trying, but I believe if it could be done with the methods we are trying it would have been
successful by now. Keep in mind that we need to get there once to close the valve, then one
more time to open it back up after the new valve is installed in the vault.
I know you three are all very busy, but please respond and let me know when we can schedule a
time to discuss a plan for going forward.

Yeah, if two vactor trucks can't keep up, we're looking at a dewatering system. Static water
table was about -7 ft when I checked last Friday and top of valve is approximately -25'
Would say we need to look at sinking a perforated casing a couple feet below the main, dump
some washed rock in bottom and pump into a tank, drain off into swale around building, then try
again. Take some pressure off.
Otherwise, we'd probably need to postpone until September/October, but deep well still likely
based on what I'm seeing and hearing.

Jason,
NW Cascade began @ 7am. Upon arrival NW removed the short 4' piece of casing and added a
10' piece. They pushed the casing down as far as possible until they felt like it was touching the
top of the watermain. NW began excavating with the vactor truck @ 8am, @ 9am the vactor
truck broke down. They got it repaired @ 11am. NW continued excavating and was able to get
below the operating nut. During the excavation NW discovered the stand pipe was broken in a
few spots. They removed the broken pieces and began attempting to install the valve nut riser
extension. They were not successful in installing the extension. The ground water is coming in
two fast. Even when using two vactor trucks they can not keep up. NW began securing the site
@ 430pm. We dumped the second vactortruck atthe decant station and called it a day.
I think it's time to step back and come up with an actual design. With the amount of water and
depth of the pipe, we should give this some real thought about how to move forward.
They will not be on site tomorrow, but we have vactor trucks on order for Thursday if we decide
to proceed.

NW Cascade began excavating to access the water valve @ Bam. NW installed a 10'piece of 24"
PVC pipe as a casing and began excavating material with a vactor truck, The vactor truck was
having a very difficult time due to large rocks. I had them excavate a little deeper with the track
hoe and we began using the vactor again. We were able to exposed the valve nut stand pipe and
we began cleaning it out. We could get down to the operating nut but every time they pulled the
vacuum pipe out ihe water and dirt rush back in and fill the stand pipe up. We decided to add an
additional 4' of 24" casing. (Sorry I mentioned 18'earlier but I was wrong). We began removing
additional material insidethe casing attempting to get down below the operating nut. I believe
this will allow the water to have somewhere to go and not plug up our stand pipe. The second
vactor truck became full @ 330pm. We decided to secure the hole and call it for the day' We will
begin in the morning removing additional material in the casing. I'll keep you in the loop as soon
as I have new information.
Our water department has decided to add a L6" butterfly valve inside the vault. They are
supplying all of the parts but they want NW to install the valve. They had to get one part from
Kentucky so they won't have everything till Thursday. Again I will keep you in the loop as things
progress.

The purpose of this action request is to provide a recap for Extra Work #10, which is the
emergency work performed at SE 288th St in Kent. This includes Vactor truck excavation,
diagnosis of issues, repairs, pavement patch and installation of butterfly valve in the vault.
I have included a pricing breakdown, as well as DPR's, invoices and receipts for everything used
to complete the work from March 21,2022 through Apil4tb,2022.
