Box 23699, Federal Way, WA, 98093 Program Description: Emergency Assistance & Resource Navigation Services (EARNS) provides housing navigation, rent, utility and emergency assistance to individuals and families in Kent. Funds moved from Year 1 to Year 2 can be utilized to support clients for rent assistance or other emergency needs. Pursuant to Section I. of the Consultant Services Agreement (CSA) executed on January 1, 2021, Consultant shall provide the approximate number of City of Kent clients with the following services for year 2 of the CSA: Service Unit Descriptions and Deliverables YEAR 2 Units for Kent Residents Contracted to Serve Service Unit 1: Voucher Description: Rent Assistance 40 Service Unit 2: Voucher Description: Emergency Assistance 20 Service Unit 3: Case Management Description: Contacts 200 First Term: 2021 Second Term: 2022 Funds Awarded: $108,000 - $ 14,000* Funds Awarded: $108,000 + $14,000* = $122,000 *remaining funds from 2021 Unduplicated Kent Clients Contracted to Serve 100 Outcomes Resulting from Service Units Achievement Rate Target Outcome Area 1: Households served are stable six months after receiving MSC services and do not return to homeless or unstable living situations. 75% Outcome Area 2: . Contract Administration -The Consultant shall notify the City, in writing, within ten (10) days of any changes in agency leadership or program personnel. -The Consultant shall provide the City with a current list of its board of directors, general or limited partners, as applicable. -In special circumstances and upon written agreement between the City and Consultant through a revised Scope and Schedule of Work, unspent funds from Year 1 of the CSA can be carried over to Year 2 or the budget otherwise redistributed between each year of the two-year CSA. The revised Scope and Schedule of Work shall be attached and incorporated into the CSA. -All records related to the CSA must be retained for three (3) years plus the current year. Service Unit Report - Data from this form will be used to track each program’s progress toward meeting the goals stipulated in the Scope of Work. It shall be submitted quarterly, no later than the 15th of the month following the end of the quarter (i.e. April 15, July 15, Oct 15, Jan 30), along with the Reimbursement Request. Reimbursement Request - This form will be filled out electronically and serve as the invoicing mechanism for payment to your agency/program. It shall be submitted quarterly, no later than the 15th of the month following the end of the quarter (i.e. April 15, July 15, Oct 15, Jan 10), unless otherwise specified. Demographic Data Report - The agency shall collect and retain the data requested on this form from the persons served through this contract. Data should be tracked in an ongoing manner and submitted annually (by January 30). Annual Outcome Data Report - Outcome data shall be submitted no later than the 30th following the end of the fourth quarter (i.e. January 30). Data should demonstrate the program’s progress toward Outcomes specified in the Scope of Work.
