PURPOSE
The purpose of this amendment is to 1) reduce $53,522 from Exhibit I for a new Exhibit I total of
$892,917, to account for the Contractor’s inability to fully expend 2021 funds within 2021; and 2)
amend scope of services to include expansion of technology support to seniors through the purchase
of computer hardware.

B. EXHIBIT CHANGES
1) Exhibit I, Section I. WORK STATEMENT: Reduce funding by $53,522 and amend the second
sentence to read:
The total amount of reimbursement pursuant to this Exhibit shall not exceed $892,917 for the
Exhibit Period noted above as shown in the funding table below.
2) Exhibit I, Section I., WORK STATEMENT, remove and replace the funding table as follows:

3) Exhibit I, Section II., PROGRAM DESCRIPTION, A, Program Activities, as amended in
Amendment 1, a new subsection 9 is added as follows:
9. Purchase and loan out tablets to enable program participants to access virtual
programming led by the Contractor. The Contractor shall:
a. Use up to a maximum of $15,182 to purchase tablets for use by eligible program
participants.
b. Distribute items purchased to participants as loaned items, to be returned to the
Contractor once the participant is no longer participating in the program.
c. Identify the specific programs these items shall be used for and develop procedures to
distribute, manage, and provide technical assistance for the use of these items as
needed.
d. The Contractor shall (i) provide proper and ample protection, and (ii) maintain
reasonable internal controls, for the tablets from damage or loss (including, without
limitation, theft) during the term of this Exhibit. The Contractor agrees to maintain the
tablets in good condition, reasonable wear and tear excepted.
4) Exhibit I, Section V., COMPENSATION AND PAYMENT, amend the 2021 quarterly amount
listed in the payment table from $70,630 to $17,108 for the Payment Period for October –
December 2021.
