Change Title Addition of Fire Services to the Phase II SOW (Version 2)
Change Request Details
Description The City of Kent requests that the Fire Services solution be added to the
scope of work for Phase II‐Full Implementation
The City of Kent requests that the Fire Services solution be added to the
scope of work for Phase II‐Full Implementation
This change order provides the cost associated to include Fire Services to the
Phase II – Full Implementation project.
The addition of Fire Services includes:
2 additional folder types and 10 additional folder sub types
New in scope folder count = 22 and sub folder count = 133
Assumptions
Phase II – In Scope: 22 (folder types); 133 (sub folder types).
Phase II – In Scope: Reports (25) based on discussions to date, it is our
understanding that this will suffice as a result of the addition of Fire Services.
Phase II – In Scope: Merge Documents (15) based on discussions to date, it is
our understanding that this will suffice as a result of the addition of Fire
Services.
Phase II – In Scope: Batches (17) based on discussions to date, it is our
understanding that this will suffice as a result of the addition of Fire Services.
If additional reports, merge documents or batches are required as a result of
upcoming analysis workshops, a separate Change Order will be submitted to
the City of Kent.

There are no interface requirements as a result of the addition of Fire
Services.
No data conversion sources included in the addition of Fire Services.
Justification The City of Kent has requested the addition of Fire Services to the SOW,
which was excluded in Section 3.3.3 ‘No Business Licensing or Fire Services’ of
the current Phase II Contract.

Cost Total cost for this change order is $26,910.00 and includes the analysis,
Interface configuration, internal QA testing, support, project management.
Schedule Vision 33 & City of Kent will update the project schedule to account for the
additional effort to provide analysis, configuration, QA, UAT support for 2
additional folder workflows
Calytera requests structuring payment as follows.
Creation of a new milestone
Milestone 2b Fire Solution Design Document and Prototype $5,850.00
Increase of the existing project milestones by the amounts stated below
Milestone 3 Folder Specification Report ‐ $6,500.00
Milestone 10 Back Office Configuration & QA ‐ $9,750.00
Milestone 17 UAT completion ‐ $2,563.60
Milestone 18 Go Live ‐ $2,246.40
