The reason stated for the removal was Civil Service Rule:. "1O.O7k: Has failed to respond to a call or certified letter from the Chief or Commission for ten (1O) working days;". The reason supporting the removal was a failure of the candidate to submit a completed Personal History Statement by the given due date. Mr. Goforth received his notice of removal from the eligibility list and followed up with the Civil Service Chief Examiner, who in turn contacted the PD Hiring / Recruiting Officer. Upon reviewing the electronic PHS, it appears that Mr. Goforth did in fact complete the PHS' however he did not "submit" it (submission of the PHS is performed using a button at the end of the PHS that says "submit" which changes the status of the PHS to "completed" on the background investigator side of the software). Since it appears that he had completed the necessary documentation to move forward in the process, I am requesting that his name be placed back on the eligibility list.
