I directed you to have these necessary forms returned to the administrative assistant by a designated date. These forms were not timely completed and returned, and you provided no sufficient explanation to justify the delay. After the above failure to follow direction, you were given clear and explicit orders by Cmdr. Kasner as to how and when to get the above required documents completed and turned in. You failed to follow those orders, which resulted in confusion and a delay in you returning to work. You then failed to report to work on 3/25/14, as ordered by me on 3/24/14. After you failed to show for duty, I contacted you by phone. During our conversation, I explained to you the seriousness of your actions, and that you had failed to follow direction on several occasions regarding the requirements to supply medical documentation. Your response was merely that you thought you had only failed to follow my direction twice. You communicated on several occasions, to both Cmdr. Kasner and myself, that you did not desire to return to work on a light duty status, even if physically able, because you would make less money on light duty than you would make while out on disability status. This could appear to be your motivation for not following command staff's direction. When we met to discuss your failure to follow direction, you stated your lack of desire to return to work on light duty was not motivated solely by financial reasons. You stated you believed light duty would impede your ability to take pain medication and conduct physical therapy. I advised you that would have been an issue for your doctor, and that your doctor could have included those restrictions on your required documentation. If your doctor determined any restriction could not support your light duty work, that determination would have been followed. You also offered an explanation for your failure to report to work as directed. You stated there was a communication problem, and hearing me wrong on 3/24/14 when I directed you back to work on 3/25/14 caused you to fail to report to work on the correct day. I do not find these explanations sufficient mitigating factors to not warrant your discipline. Due to your failure to follow my clear direction on several occasions in violation of department Policy 13.1.1.T, Insubordination, you have been issued this Written Reprimand as discipline. Any further incidents of this nature will incur additional progressive discipline.
