AMENDMENT NUMBER FOUR
TO THE FOOD SERVICE AGREEMENT BETWEEN
CITY OF KENT
AND A'VIANDS, LLC.
This Amendment Number Four is made and entered into by and between Mill Creek Senior
Center (hereinafter referred to as "Client"), whose address is 600 East Smith Street, Kent,
Washington 98030, and Consolidated Food Management, Inc., now known as A'viands,
LLC (hereinafter "A'viands"), whose address is 1751 West County Road B, Suite 300,
Roseville, MN 55113.
WHEREAS, A'viands and Client are parties to a Food Service Management Agreement, as
amended (hereinafter referred to as "Agreement") effective February 15, 2016.
WHEREAS, both parties now desire to amend said Agreement upon the terms and
conditions stated herein.
NOW, THEREFORE, the parties, intending to be legally bound hereby, mutually agree as
follows:
1. Section 3.1 Term of the Agreement: This Agreement shall be extended for a
one (1) year period commencing February 15, 2020 and ending February 16,
2021, as allowed per the terms of the original Agreement.
2. Section 9.1 Pricing Structure: Effective February 15, 2020, the price per meal
shall be adjusted from $6.60 to $6.80.
This Amendment Number Four shall be effective as of February 15, 2020. All other terms
and conditions of the original Agreement, as modified from time to time, shall remain in full
force and effect unless otherwise amended as provided in the Agreement.
