This Change Order amends the above-referenced contract; all other provisions of the
contract that are not inconsistent with this Change Order shall remain in effect. For valuable
consideration and by mutual consent of the parties, the project contract is modified as follows:
1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add
additional work or revise existing work as follows:
In addition to work required under the original Agreement and any prior
Amendments, Contractor shall provide all labor, materials, and
equipment necessary to:
Remove and replace three existing sewer cleanouts to allow for the
installation of CIPP liners.
2. The contract amount and time for performance provisions of Section II "Time of
Completion," and Section III, 'Compensation," are hereby modified as follows:

In accordance with Sections 1-04.4 and 1-04.5 of the Kent and WSDOT Standard
Specifications, and Section VII of the Agreement, the Contractor accepts all requirements of this
Change Order by signing below. Also, pursuant to the above-referenced contract, Contractor
agrees to waive any protest it may have regarding this Change Order and acknowledges and
accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order, including,
without limitation, claims related to contract time, contract acceleration, onsite or home office
overhead, or lost profits. This Change Order, unless otherwise provided, does not relieve the
Contractor from strict compliance with the guarantee and warranty provisions of the original
contract, particularly those pertaining to substantial completion date.
All acts consistent with the authority of the Agreement, previous Change Orders (if any),
and this Change Order, prior to the effective date of this Change Order, are hereby ratified and
affirmed, and the terms of the Agreement, previous Change Orders (if any), and this Change Order
shall be deemed to have applied.
The parties whose names appear below swear under penalty of perjury that they are
authorized to enter into this contract modification, which is binding on the parties of this contract.
3. The Contractor will adjust the amount of its performance bond (if any) for this project
to be consistent with the revised contract sum shown in section 2, above.

become effective on the last date written below.

Approval of this change order is being sought to address expenses and credits that have arisen and are necessary to
complete the referenced project.This change order includes the following Change Proposal Requests(CPRs)which are
described as follows:
CPR 003:This CPR addresses the removal and replacement of three (3) sewer cleanouts within the general
extents of the project.The existing cleanouts are to be removed and disposed of by the contractor so that CIPP liners
can be installed in the existing pipe, as described in RFI 001. Following installation of the CIPP liners, each of the
cleanouts is to be replaced is accordance with Kent Standard Plan 4-7. Backfill and surface restoration are incidental to
the cleanout replacement and shall be in accordance with Kent Standard Plan 6-65. Existing survey monuments, if
disturbed, are to be replaced in accordance with Kent Standard Plan 6-72 and applicable local regulations.
Cost of CPR 003: $27,400.00
Staff has participated and reviewed all changes proposed and recommends moving forward for approval in this change
order.

to receive a quote for the city to install manholes in its place. This would provide an upstream access point for
camera insertion for maintenance, while increasing crew safety when installing the liner vs. working from a
shores access pit.

Please see the revised pricing for cleanout replacement attached. Note that I mis-remembered the cost
of the monument survey when we spoke yesterday, it is$1500 each, not$1500 for all three, so the total
cost is$27,440 with markup but before tax.
Please confirm if the proposal is acceptable as soon as possible since Insituform needs to proceed
immediately to avoid impact to the schedule.

- Work involved completing the excavation and removal of cleanout K49 started yesterday (8/12) and
excavation and removal of cleanout at the end of 249th Pl. S. required to support lining of the associated
sewer mains.
- 9:45 spoke by phone with Laser (Brent Lane), where he requested a representative from the City Parks to
dept. meet at Linda Heights Park to discuss the removal of hog fuel and restoration of the park access. I
set up a meeting with Scott Swanson for 8/14 at 7am. Items also discussed involved: (1) Laser providing a
new cleanout base cover and covers, (2) cleanout backfill will be 5/8inch recycled material, (3) the cleanout
concrete collar will match existing grade per city std. detail, (4) process for billing hog fuel, I directed Brent
to speak with Insituform (Mike Morris).
- Per email from Insituform (Mike Morris) P&D Logging will be on site tomorrow (8/14) to remove the two
identified trees along the access road.

LASER General Notes:
- Work started at 7:00am and completed at 4:30pm.
- Work involved backfilling, compacting and forming concrete collars for cleanouts K37 and K49.
- Backfill was placed in 1-2 ft lifts and compacted with the hoe-pack. Where the hoe-pack was unable to be used, a
jumping jack and 6-12 inch lifts were used.
-After requests from contractor to consider alternative cleanout collar shape, MS approved a round concrete collar
vs square. The alternative was discussed and approved by the City.
- Meeting at Linda Heights Park was rescheduled from 7:00am to 10:00am. Attended by Erik Johnson (Laser), Scott
Swanson (City of Kent- Parks Dept.) and myself. The goal was to ensure Laser knew how the City wanted the park
restored. Below are the items requested by the city to restore the park to preconstruction condition:
•All the hog fuel needs to be removed.
• Any soil and sod that has been disturbed needs to be leveled.
• 2-3 inches of top soil (per spec) needs to be placed along the impacted area.
• Grass seed (per spec) needs to be spread along impacted area and incorporated into the top soil.
•After restoration has been completed, the irrigation system will be tested and any damage will need to be repaired.

- Asphalt was compacted and leveled using a combination of a walk behind whacker and driven roller.
