SAFETY COMMITTEE MINUTES
May 16, 1989
Present: Bruce Lipp, Barbara Keohane, Joe Sams and Lori Brown
FIRE
Incident: During a demonstration, a firefighter rain around the
vehicle to turn off a valve and cut his hea& on the end
of a ladder in the down position.
Comments: The Committee felt that this was a preventable accident
and could have been avoided with a helmet.
PARKS
Incident: Employee attempted to dump a basket of balls and injured
his arm. Possibility of the use of improper lifting
procedures.
Comments: Inadequate information was given on the accident report
so a determination was not made regarding this particular
accident. The supervisor has been contacted since the
last meeting and clarified that the employee had not been
using proper lifting procedures and has been given
instructions for lifting heavy loads. The Committee will
again review this accident in June.
PARKS
Incident: While scrubbing walls and ceilings of a building,
employee overextended arms and strained neck.
Comments: This accident was considered preventable by the
Committee. It was advised that the employee move the
ladder more often when cleaning ceilings.
POLICE
Incident: Injured leg when jumping over the six foot wall at the
Academy.
Comments: Proper procedures were used in this particular incident,
so the Committee determined that this „i¢ c; ent was non-
preventable.
POLICE
Incident: Employee struck foot on a partially open file drawer.

Comments: This was considered to be a preventable accident and it
was advised that the employee should be more careful, and
the office staff should close drawers that are not
currently in use.
POLICE
Incident: Officer was shooting at City of Kent ranges for weapons
qualification when he was struck on the outside edge of
the right cheekbone by a bullet
fragment.
Comments: The Committee was very concerned about this incident and
asked that Chief Frederiksen be contacted , to determine
why the range is apparently unsafe. According to Chief
Frederiksen, unauthorized persons had entered the range
and were using weapons that damaged the metal plate. The
lane was closed down when the damage was discovered and
not opened until it had been fixed. In the future, the
Range Master will be checking for damage on a regular
basis.
Also discussed was the possibility of obtaining handouts regarding
poison ivy and other hazards of this nature. Joe .Sams will be
investigating this issue and providing the City with materials.
