SAFETY COMMITTEE MINUTES
MAY 17, 1988
Present: Ken Cooper, Bruce Lipp, Jennifer Carey, Don Fronsdahl , Kevin Ellis,
Jim Ausburn, Joe Sams, Peggy Todd, Becky Fowler.
The following items were discussed:
AUTOMOBILE ACCIDENTS:
Building Department
INCIDENT: While employee was backing out of parking stall at Johnny's Market
a vehicle driven by another person backed out across from him. The
right rear bumper of city vehicle dented the left rear panel of
other vehicle, with no damage to the city vehicle.
COMMENTS: I Employee should be more aware of other vehicle, careful and use
proper backing procedures.
Sewer Department
INCIDENT: While employees were picking up barricades from job site and moving
the truck, the first barricade fell breaking rear window on truck
#230.
COMMENTS: Committee agrees with the Department's actions advising employees
of proper loading procedures. Also recommended having screening
installed on the outside of back window of all City pick-up trucks
for protection.
Park Maintenance Department
INCIDENT: No parking on street so parked truck on grass. While mowing the
grass, came too close when passing the truck. Grass basket caught
grill and tore the corner off.
COMMENTS: Safety Committee advises employee to park truck on or across street.
Street Department
INCIDENT: Employee was looking down at the ground while sealing an asphalt
patch when employee struck him with side of mirror on truck #84
while attempting to maneuver truck around him to park.
COMMENTS: Committee advises employee to be more aware of surroundings.
Public Works Department
INCIDENCE: Had just exited his work vehicle when a gust of wind arose at that
precise moment and blew a foreign object into, hi,s eye.
COMMENTS: Safety Committee could not fathom any preventAti a Measure employee
could have taken to avoid this.
Fire Department '
INCIDENCE: Basketball injury. A slight bump between two employees occurred.
One turned his ankle while changing directions. The bump was not
instrumental .
COMMENTS: Employee may try wearing high top shoes to help prevent turning of
the ankle.
Fire Department
INCIDENT: The firefighter was performing fire suppression activities when he
slipped and fell on wet concrete, bruising his knee.
COMMENTS: Be more careful of wet concrete. No preventative measure could be
taken in this case.
Corrections Department
INCIDENT: Officer standing inside the day room door, asked ail inmates to
cell in and while backing out of room turned, whw their name was
called. The door slammed on left hand, little finger.
COMMENTS: Employees need to be more careful around automatic doors and more
aware of the dangers inherent when they are inattentive during
routine operations. ,
Fire Department
INCIDENT: As employee was getting into jump seat on fire truck, struck knee
on the brass coupling causing injury.
COMMENTS: Employee should be more careful during routine inspections, slow
down. Safety Committee recommended padding areas of fire truck
around jump seat.
Fire Department
INCIDENT: " After working out at Lake Fenwick Park, turned and stepped into a
small hole that was grown over with grass and twisted their right
ankle.
COMMENTS: Employee should be more careful and watch where they are walking.
Parks Maintenance Department
INCIDENT: While Shoveling out tree basins in Burlington Greens Park pulled
knee muscles.
COMMENTS: Use of proper digging procedures and training for reporting of on
the job injuries.

Street Department
INCIDENT: While pulling branches out of trees, a branch f I 'striking the
employee on the side of the head by left eye.' �
COMMENTS: Safety Committee recommends work hats, glasses, and vests for
protection while completing these types of jobs.
