SAFETY COMMITTEE MINUTES
March 15, 1988
Present: Jack Fingold, Jim Ausburn, Ken Cooper, Jennifer Carey, Beth
Remington, and Peggy Todd
The following items were discussed.
Police Department
INCIDENT: While walking to the women's locker room, the carpet remnant on the
floor moved under employee's foot causing employee to slip. While
employee was trying to catch herself, she struck her left elbow
against the brick wall and then fell down to her knees. No injury
resulted, however her left elbow was sore after the incident. The
department has removed the carpet remnant from this area as a
corrective measure.
COMMENTS: The Committee agrees with the department's action in removing the
carpet remnant.
Water Division
INCIDENT: Employee stepped off of back hoe and went around front of bucket to
assist a crew member and stepped into a one foot deep hole
straining his lower back.
COMMENTS: The Committee agrees with action taken by the division to inform
employees to be more aware and careful in their work environment.
Parks - Special Populations Facility
INCIDENT: While employee was coaching Special Olympics Basketball and running
drills on defense, one of the athletes put his hand up in defense
hitting the employee in the left eye with one of fingers. Employee
was wearing contacts at the time and experienced pain in his eye.
COMMENTS: Committee would recommend that employee's be more careful when
participating in sports activities and possibly wear eye protection.
Fire Department
INCIDENT: While employee was supervising the overhaul in a house which
required the removal of paper fiber insulation that contained
smoldering embers, the employee was exposed to light smoke
conditions for approximately two hours.

COMMENTS: Appropriate action would be for employees to wear appropriate
breathing apparatus when smoke is apparent. Proper procedures
should be followed when this type of atmosphere is present or when
atmosphere changes.
Engineering
INCIDENT: While employee was opening a gate valve, his hand, slipped causing
his elbow to hit the gate valve key.
COMMENTS: Employee should be careful and ask for assistance when a valve is
too hard to turn. A suggestion by The Committee would be to have a
valve program established to maintain valves.
AUTOMOBILE ACCIDENTS
Fire Department
INCIDENT: While employee was maneuvering apparatus 716 between engine 72 and
parked cars, he struck the bumper guard of another vehicle with the
left rear bumper of the engine.
COMMENTS: Employees should be more careful and use proper procedures when
operating vehicles in limited space areas. The Committee agrees
with the department's action whereby employees have been spoken to
about avoiding tight situations and the use of an outside observer.
FIRST AID TRAINING
The Committee will work with the fire department in setting up first aid and CPR
training for city employees during the year 1988.
INVESTIGATION AND REPORTING PROCEDURES
The Committee discussed the need for all supervisors to do a thorough job of
investigating accidents and incidents when compl,eting their report forms. These
report forms are reviewed by The Committee monthly at our safety meetings.
THIRD TAIL LIGHT
The Committee again has requested that the possibility of having a third tail
light installed on city vehicles by the equipment rental division be explored.
The Committee feels that the installation of this third tail light will help in
reducing the number of rear end accidents that employees are sometimes involved
in.
