The following incidents/accidents and injuries were discussed. FIRE Incident: While evaluating a patient at an aid scene, the family dog went for the employee from under the table without barking and bit the shin of his lower left leg. Comments: Non-preventable. While entering home, ask about loose animals. Incident: Employee was at WA. ST. Fire Academy Drilling. Was removing a kink from a 5 inch supply line when a water hammer ripped the hose from his hands. Upon recoil, it pinched his left leg, throwing him forward. He landed, knee was twisted with the hose on his leg and his body on the hose. Comments: Non-preventable. Is 5 inch hose a two person job? Does not matter how many people are there. Non- preventable. Incident: Employee was moving a scaffold when he fainted and hit his head. Was indoor. Comments: Non-preventable. Incident: Employee was on top of hose bed of apparatus 713 . He raised the hose bed cover and felt a vertebrae shift. Comments: Preventable. Use proper lifting procedures and give enough room to lift. ENGINEERING Incident: Employee was opening vault door to put down ramp. Ramp fell on foot. Comments: Supervisor report required. Karen will send report to Personnel as soon as she gets back from vacation. CORRECTIONS Incident: Employee was escorting inmate from one living unit to another. Was carrying toiletries in left hand. Stopped at dayroom door to ring intercom. As employee was doing this, a razor dropped. Cut finger as he was catching the razor. Comments: Preventable. Should use storage container, eg. baggy. Incident: Employee was stepping to the lobby window to talk to a work release inmate. Stepped off the two stairs and landed on the inner part of right foot. Heard a crack/pop and then felt pain in right foot. Comments: Preventable. Make steps more noticeable, eg. tape, lights. PUBLIC WORKS Incident: Employee was stepping off grader onto small rocks. Twisted right knee. Felt sharp pain instantly. Comments: Non-preventable. Use care when stepping off equipment. Incident: Employee was clearing brush and shoveling dirt of the side of the road when he noticed a sharp pain in the right side of his lower hip and back. Comments: Need supervisor's report. Jonnie Nall will send report to Personnel ASAP. Incident: While changing chlorine tanks, employee was using a valve wrench to open the valve. Was hitting the wrench with left hand to open the valve. After a day or so, hand started to hurt and swell. Comments: Preventable. Use rubber mallet. Incident: Employee was backing truck with rebar on rack sticking out the back. Rebar hit and broke window in warehouse door. Comments: Preventable. Use proper stopping and backing procedures. Incident: Employee was attempting to pull mower #103 out of ditch with truck #429 and chain. Attached chain to hooks on frame of truck #429 . Started to pull tractor out of ditch. Bolts sheared on left side, holding hook to frame. Pulled off front bumper and left side, and bent bumper and dented right front fender. Comments: Preventable. Use proper towing procedures. POLICE Incident: Employee was stopped in his vehicle westbound on S. 240 at Pacific Highway South. An oncoming vehicle eastbound crossed Pacific Highway South and was struck by a northbound vehicle. The eastbound vehicle lost control and struck employees' vehicle. Comments: Non-preventable. Incident: While in the process of checking a rifle to see if it was loaded, the rifle discharged, ejecting the rifle bolt which severely cut employee's right hand and embedded foreign objects into right eye. Comments: Preventable. Use proper procedure in checking loaded guns. Incident: Employee A was near another employee (B) when the rifle that employee B was trying to check exploded. The explosion occurred in a small, jail like cell. As a result, the noise was very loud. Employee A complained of ringing in ears and some pain. Comments: Non-preventable. Incident: Employee was on foot/K-9 pursuit of suspect. As he was going over a chain-link fence, the top wire edge punctured palm of his left hand. Comments: Non-preventable. FINANCE Incident: Employee was trying to untangle a dog on a chain, which wrapped around a tree when the dog bit employee on the wrist. Comments: Preventable. PARKS Incident: Employee parked in parking lot. Thought she had car in Parking shift. Walked away. Car rolled across parking lot and hit shrub bed and tree. No other person or vehicle involved. Comments: Preventable. Driving techniques. OTHER BUSINESS: PUBLIC WORKS - OPERATIONS YARD Entrance/Exit Gates. Golf Complex is exiting small equipment i.e. lawn mowers from the entrance gate to the Shops yard. The lawn mowers are not licensed for public street usage and employee safety is an issue with the incorrect gate being utilized.
