SUBJ: VSRB Accident Review/accident 93-6562

I have reviewed the Vehicle Safety Review Board recommendations,
accident reports, your previous history involving vehicle
accidents, and it is my determination that this is classified as an
accident which was preventable. As for sanctions, I have decided
that a written reprimand is the most appropriate action in this
case. If you have any questions or information that might have
further bearing on this investigation or my decision please contact
me no later than December 15, 1993 at 1700 hrs . Otherwise my
decision stands and a copy of this memorandum will be placed in
your personnel file with a copy forwarded to the Human Resource
Department.
