SUBJECT: On-Duty "IN-HOUSE^ Accident 92-2827 (03-27-92)
On 03-27-92 you were involved in an On-Duty traffic collision
with patrol vehicle #354 . The location of the incident was the
driveway at 21645 64th Ave. S. . As set forth in Policy N-03 that
collision would be classified as an "Incident"
Policy K-20 mandates a determination for . Incidents as
preventable/non-preventable orjustifiable/unjustifiable. A review
of the case report and supportive documents in 92-2827 lead to the
finding that this collision was "Preventable" . You were making a
90 degree turn traveling between gate posts that were 12 feet 5
inches apart. The case report indicates a distraction by the sun.
Inattention and/or lack of caution resulted in this collision.
Since Policy N-03 and K-20 have come into affect this is
your only "Incident" It is however the third collision you have
been involved in since October of 1991. (10-28-91 CR91-9344 ,
03-27-92 CR92-2827 , and 07-31-92 CR92-6459) This document serves
only to satisfy department policy regarding classification and
tracking of vehicle collisions relating to this"Incident" . It
will be maintained in your personnel file. This is NOT a
reprimand. The two additional Accidents (91-9344 and 92-6459) are
matters for the Vehicle Safety Review Board. It should be noted
that accident 91-9344 occurred prior to Policies N-03 and K-20
and/or the formation of the Vehicle Safety Review Board.

SUBJECT: Attached "Incident Reviews" Per Policies N-03 & R-20
Attached are individual Incident by Incident reviews of
vehicle collisions involving the above listed officers, that by
Policy N-03 are classified as "Incidents" not Accidents. Each
named officer has received this cover memo and the Incident review
of the collision they were involved in. I have also forwarded a
set to Chief Crawford
To each of you I offer my apology (not an excuse) for the
• delay in forwarding these reports. The nature of these collisions
have been made known to Sgt. Woods and are being incorporated into
the EVOC/ABS course presently being taught.
