CONTRACT NAME & PROJECT NUMBER: Surin-gs and- Pump_Station No, A_ Electrical Upgrades ORIGINAL AGREEMENT DATE: June 27, 20I.8 This Amendment is made between the City and the above-referenced Consultant or Vendor and amends the original Agreement and all prior Amendments. All other provisions of the original Agreement or prior Amendments not inconsistent with this Amendment shall remain in full force and effect. For valuable consideration and by mutual consent of the parties, Consultant or Vendor's work is modified as follows: 1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add additional work or revise existing work as follows: In addition to work required under the original Agreement and any prior Amendments, the Consultant or Vendor shall: Continue to provide on-call services during the construction of the Kent Springs and Pump Station No. 4 Electrical and Standby Generator Upgrade Project. APPROVED AS TO FORM: (applicable if Mayor's signature required) Kent City Clerk Kent Law Department AMENDMENT - 20F2 EXHIBIT A Scope of Work City of Kent I Amendment No. 1 Kent Springs and Pump Station No. 4 Electrical and Standby Generator Upgrades Services During Construction November 2018 Background This Scope of Work includes additional assistance by RH2 Engineering, Inc., (RH2)with specific services during construction(SDC)tasks,as directed by the City of Kent(City),during the construction of the Kent Springs and Pump Station No.4 Electrical and Standby Generator Upgrades.The City will continue to perform the majority of the SDC tasks for this project.This Scope of Work details the SDC tasks that will be performed by RH2 on an on-call basis. Task 3— Additional Services During Construction On-Call Support Objective: Provide continued on-call services during construction of the project. Approach: 3.1 Review Submittals: Review and approve or reject, if necessary remaining submittals that the City requires 11112's assistance in reviewing. 3.2 Respond to Technical Questions from the City: Provide discussion,via telephone or email, with City to respond to questions that arise during project construction and testing. 3.3 Review and Respond to Requests for Information (RFIs)Submitted by the Contractor: Provide review and responses to RFIs submitted bythe contractor to the City. 3.4 Provide Testing and Startup Support:Provide technical support during testing,startup, and observation. This support includes up to one(1)site visit to both the Kent Springs and Pump Station No. 4 sites to review the completed work and assist the City in developing a project punch list. RH2 is not responsible for site safety, nor for determining means and methods or directing the Contractor in their work. Assumptions: o RH2 will review submittals and respond to technical questions and RFIs as supplied by the City and provide services up to the fee amount allocated for Task 3 in the Fee Estimate (Exhibit 8), If the fee amount for Task 3 is reached, RH2 will notify the City, and if further assistance is required above and beyond the fee amount allocated forTask 3, the City and RH2 will mutually agree upon an additional fee amount for continued on-call support.
