This Change Order amends the above-referenced contract; all other provisions of the
contract that are not inconsistent with this Change Order shall remain in effect. For valuable
consideration and by mutual consent of the parties, the project contract is modified as follows:
1. Section I of the Agreement, entitled "Description of Work," is hereby modified to
add additional work or revise existing work as follows:

This Change makes the following revisions to the Contract Documents:
Item 1: Plan sheet T1 of T3 - On the NE corner, the foundation for
Pedestrian Push Button (PPB) 8 is deleted. Mount the PPB on the
existing signal pole. An Extension shall be installed to mount the
button within 10" of the face of pedestrian curb. The foundation of PPB
9 is shifted back behind the pedestrian curb about 12", and an
extension shall be installed to mount the button within 10" of the face
of pedestrian curb.
Item 2: Plan Sheet T1 of T3 - Make the following changes to the
induction loop vehicle detectors: Shift departure loops N1 and N2
south, and route the home run the junction box at -8+25 LCL. Shift
Loops 231B, 221B, 211B north approximately 4'. Add 2 loops in the
dedicated right turn lane in the East Bound direction. Leave existing
left turn loops 111A, 121A, 111B, 121E in place. Change loops 611A,
621A, 611B, 621B from circular loops to square 6x6 loops. See FWD
004 for a drawing of these changes.
This change has been negotiated and all parties agree there are no
cost or time impacts to the project. All work associated with this
change are being paid for under the Lump Sum bid item 6000, and as
this is a no-cost change the item will be paid in full.

In accordance with Sections 1-04.4 and 1-04.5 of the Kent and WSDOT Standard
Specifications, and Section VII of the Agreement, the Contractor accepts all requirements of this
Change Order by signing below. Also, pursuant to the above-referenced contract, Contractor
agrees to waive any protest it may have regarding this Change Order and acknowledges and
accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order,
including, without limitation, claims related to contract time, contract acceleration, onsite or
home office overhead, or lost profits. This Change Order, unless otherwise provided, does not
relieve the Contractor from strict compliance with the guarantee and warranty provisions of the
original contract, particularly those pertaining to substantial completion date.
All acts consistent with the authority of the Agreement, previous Change Orders (if any),
and this Change Order, prior to the effective date of this Change Order, are hereby ratified and
affirmed, and the terms of the Agreement, previous Change Orders (if any), and this Change
Order shall be deemed to have applied.
The parties whose names appear below swear under penalty of perjury that they are
authorized to enter into this contract modification, which is binding on the parties of this
contract.
3. The Contractor will adjust the amount of its performance bond (if any) for this
project to be consistent with the revised contract sum shown in section 2, above.

I. PROPOSED CHANGE
This Change makes the following revisions to the Contract Documents:
Item 1: Plan sheet T1 of T3 - On the NE corner, the foundation for Pedestrian Push Button (PPB)

Ii. REASON AND BACKGROUND FOR CHANGE
Item 1: During excavation for the foundation of Pedestrian Push Button #8, the existing signal
pole foundation was found to be in conflict with the location of the new PPB foundation. The City
decided to delete the foundation. Also during excavation, an existing conduit was found in
conflict with the location of the PPB foundation #9. The City directed the contractor to install the
foundation behind the pedestrian curb to clear the conduit. To meet ADA reach requirements,

extensions are needed to bring the buttons within 10" of the face of the pedestrian curb.
Item 2: The departure loops N1 and N2 were shifted south and home runs were re-routed to a
junction box at 8+25 LCL at the City's direction. This is due to conduit capacity issues in the
conduit run between the junction box at 8+25 LCL and 9+35 LCL. Loops 23113, 22113, and 211B
are shifted north at the City's direction to avoid poor roadway conditions and existing utilities.
The two loops added to the eastbound dedicated right turn lane are done at the request of the
City to replace an existing square loop that was damaged during per plan pavement removals on
the SW corner. The contract documents did not call for replacement of the damaged loop.
Existing loops 111A, 121A, 111B and 121E are remaining at the direction of the City because
they are operational and the existing pavement was determined to be too rutted to successfully
install new traffic loops. Traffic loops 611A, 621A, 611B, and 621B are being changed to square
type loops due to the existing roadway being too rutted for the successful installation of circular
loops.

2/1/2017—During excavation for the PPB foundations on the NF corner,conflicts with an existing signal base and conduit
require deleting one base and moving another.Verbal direction given to the contractor.
2121117—The Contractor provided RFI 008,requesting information on PPB extensions to keep within ADA reach limits.
2/22/17—The City responded accepting their proposed fix,and requesting a cost proposal.
3/28/17—The contractor provided a proposal of$700 for the addition of the PPB extensions.
4/6/17—The City requested a credit proposal for the deletion of the PPB foundation,
4/17/17—The Contractor responded that they proposal a no cost change,swaping the PPB foundation deletion for the
extensions.
5/15/17—The City responded, provided their estimate and requesting a total credit of$750 for the deletion of the PPB
foundation and the addition ofthe button extensions,
5/16/17—The Contractor responded, agreeing to the credit proposal of$750.
Item 2—Vehicle Detection Loops
5/17/17—DBE Electric,City of Kent Signals,and KBA met onsite to discuss vehicle detection loop,conduit capacity,and existing
roadway condition Issues. We also discussed potential costs of the issue,and all parties agreed there was a little extra work
involved. DBE verbally suggested offsetting the cost of the PPB extensions credit with this work into a no cost change.
5/23/17—Independent cost estimate totals$482.27 for the additional work.
5/30/17—The City issued FWD 004,directing the contractor to make all of the vehicle detection loop changes discussed.The
FWD also notes an anticipated no cost change will be written,

Still have to install Campbell AGPS per invoice on MOH request)
Any mounting hardware (—$50)
Labor/equipment for install ofAGPS on signal pole (Estimated 4 Elect. hrs+Truck, $400)
Total Add/remaining work: 700 F515+50k400=$1,665

Our price includes all labor at the standard daytime rate, materials, equipment, and allowable markup

0730 RCNW began demo of hard scale at the NW corner. I spoke with Steve Olson who requested PPBs be surveyed
prior to the top back of curb,
080C Silver streak truck is on site, RCNW exporting concrete debris from the NW corner.
Received a call from Steve Olson regarding the placement of the ped push buttons at the NE corner.Both push
1015 buttons have conflicts with existing items.The southern most ped button is over the existing traffic signal conduit
and the northern PPB may be in conflict with the existing signal pole base.DBE Electric has installed extensions,
similar to those used for pedestrian srgnal_head son other projects and su gg ested;using those.
1030 I spoke with Eric Connor and Susanne smith about the PPB locations.They are ok with moving them and using the
extensions.

Arrived onsite and confirmed the southern PPB on the NE corner was centered on the existing signal conduit It is
1100 being installed behind the conduit. DBE is using a 2"diameter sono tube for a form, and the plans call for a 1'-6"
square base.This does push the base back 6'further than it would have it if it was the square base shown.
1115 RCNW continuing the remove sidewalk adjacent to the rockery.Survey is onsite, shooting in ped push buttons.
1200 Survey left the site for the day. DBE completed installing conduit and the sono tube form for the southern PPB on
the NE corner. RCNW be ri i bir ear laine, ulr t hu a ,rlli rt tPw L ue cart the rockery.
1300 RCNW removing concrete curb and gutter at the at the driveway for key bank,and noted there was about 15'
where the gutter was 12"thick.When it thinned back too the typical 6"depth,there was asphalt under the curb.
1330 Got a call from Ed Giraud who was onsite. I had just left.
