For valuable consideration and by mutual consent of the parties, the project contract is modified as follows: 1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add additional work or revise existing work as follows: In addition to work required under the original Agreement and any prior Amendments, Contractor shall provide all labor, materials, and equipment necessary to: 1. Extend the handrail 18 LF (three panels) south from STA 73+36 RCL to overlap the existing pedestrian wall on the bridge for safety and aesthetic reasons. 2. Terminate the handrail on either side of the luminaire pole located at STA 72+38 RCL to allow approximately 6 inches of additional sidewalk width. 3. Revise the handrail at the PSE vault located at STA 73+05 LCL from a removable straight handrail in front of the vault to a permanent handrail wrapping around the vault allowing for easier access and provides fall protection. 2. The contract amount and time for performance provisions of Section II "Time of Completion," and Section III, "Compensation," are hereby modified as follows: Original Contract Sum, $5,819,698.74 (including applicable alternates and WSST) Net Change by Previous Change Orders $130,089.29 (incl. applicable WSST) Current Contract Amount $5,949,788.03 (incl. PROPOSED CHANGE This Change makes the following revisions to the handrail shown on Contract Drawing 30 of 37: 1. Extend the handrail 18 LF (three panels) South from STA 73+36 RCL to overlap the existing pedestrian wall on the bridge. Set the southernmost base plate for the end post just inside (east side) the wall. Allow for expansion in the railing segment crossing the bridge deck expansion joint. 2. Terminate the handrail on either side of the luminaire pole located at STA 72+38 RCL. Locate the handrail base plate as close to the luminaire pole as necessary to prevent the passage of a 4" sphere between the luminaire pole and the handrail. 3. Revise the handrail at the PSE Vault located at —STA 73+05 LCL from a removable straight handrail in front of the vault to a permanent handrail wrapping around the vault and mounting to its outside edge. This Change also adds a bid item to provide compensation to the Contractor for additional costs associated with this Change. 11. REASON AND BACKGROUND FOR CHANGE Item 1. Contract Drawing 30 of 37 calls for the handrail to match the existing railing at 73+36.93. The existing railing was scratched, dented, and had bases and anchors that were not as strong as the new railing being installed. For both safety and aesthetic reasons, the City elected to have the remained railing, approximately 15 LF, removed and replaced with the new railing being installed under this Contract. Item 2. Contract Drawing 30 of 37 calls for the handrail to pass in front of the luminaire at STA 72+38. The City elected to terminate the handrail on either side of the luminaire, which provided approximately 6 inches of additional sidewalk width. Item 3. Contract Drawing 30 of 37 calls for the handrail to utilize the existing base plates on the PSE vault at STA -73+05 LCL, and to make the railing removable. Between the project bid and start of construction, PSE replaced the vault lid, removing the existing base plates. While redesigning the railing to work with the new lid, the City elected to install a railing around the 1 REV. DATE: 4/01/10 • FILE NO: 200.2 KENT outside edge of the vault. Contract No: 13-3004 Owner: City of Kent Federal Aid No: STPUL-1071(006) Subject: Change Order 01X Chronology of Events Message 7/29/2015—Handrail Shop Drawing and Enginee-ed Calculations approved by City. 9/24/2015—RFP 005 is issued to Marshbank Construction, Inc(MCI),proposed changes included terminating the handrail on either side of the luminaire pole at 72+38 RCL and the addition of 18 LF of railing by the driveway at 70+35 RCL.The RE team put together an Independent Cost Estimate(ICE). 10/6/2015—MCI Serial Letter 011 sent to the City regarding the proposal requested in RFP 005. 10/19/2015—The City issued FWD 012,directing MCI to perform the work noted in RFP 005 after reviewing the pricing proposed in MCI Serial letter 011. 11/20/2015—the City issued RFP 007 to MCI requesting a proposal to install the handrail on top of the new PSE vault lid, wrapping around the outside of the lid instead of a removable straight section in front of the vault. 1/27/2016—the RE team completes the ICE for RFP 007 2/4/2016—the City issues FWD 016 to add 18 LF of railing to the South termination point on the West side of Central. 3/29/2016—MCI provided Serial Letter 024 and 024,responding to RFP 007 and FWD 016. MCI and stating the payment for the additional 18 LF of railing was not sufficient due to small batch pricing. 4/4/2016—The City reviewed the pricing provided in MCI Serial Letter 024 and 025,and agreed to the costs. 5/11/2016—MCI submitted SUB 028 shop drawings for the handrail at the PSE Vault. 5/18/2016—The City reviewed the submittal,and noted the handrail anchorage is different from that originally submitted and evaluated by the handrail engineer of record.The submittal was returned at Amend and Resubmit. 6/13/2016—MCI provided an additional price quote,for the redesign of the handrail as the one provided by the City in RFP 007 would not work. 6/17/2016—MCI provided a conceptual drawinF'of the proposed new mounting design for the handrail at the vault. 6/22/2016—The City reviewed the new mounting design with PSE,who agreed to allow the mounting of the railing to their vault as proposed.The RE Team also provided an ICE for the additional costs.The City verbally agreed to MCIs cost proposal and verbally directed them to proceed. 7/6/2016—MCI submitted SUB 032—Revised PSE Vault Handrail Layout and Calcs to the City. 7/13/2016—The City reviewed SUB 032 and approved the submittal. Handrail Revisions RFP 005 Serial Letter No. 11 Cameron, RFP 005 outlined changes to the handrail located on the south end of the project. Item one proposes adding 18 if at STA 70+53 to 70+35 RT. For the additional footage, this work can be performed at existing bid item pricing for B1-1295. Item two proposes adding two end sections at 72+38 RT due to a conflict with an existing luminaire pole. We propose an additional $350 per each end section for labor and materials to revise the handrail layout. This cost will cover the added fabrication and installation time required to build this break in the handrail system. Please review and let us know what questions you have.
