CONTRACT NAME & PROJECT NUMBER: Armstrong Springs Motor Control Center and Generator Upgrade ORIGINAL AGREEMENT DATE: May 4, 2015 This Amendment is made between the City and the above-referenced Consultant or Vendor and amends the original Agreement and all prior Amendments. All other provisions of the original Agreement or prior Amendments not inconsistent with this Amendment shall remain in full force and effect. For valuable consideration and by mutual consent of the parties, Consultant or Vendor's work is modified as follows: 1. Section I of the Agreement, entitled "Description of Work," is hereby modified to add additional work or revise existing work as follows: In addition to work required under the original Agreement and any prior Amendments, the Consultant or Vendor shall: Provide bid document preparation and construction support for the project. For a description, see the Consultant's Scope of Work which is attached as Exhibit A and incorporated by this reference. 2. H2) with specific SDB and SDC tasks, as directed by the City of Dent (City), and preparation of additional updates to the plans and specifications at the direction of the City, based on 100-percent review continents from the City.The City will perform the majority of the services during bidding (SDB) and services during construction (SDC) tasks for this project.This Scope of Work details the 100-percent design updates,and the SDB and SDC tasks that will be performed by RH2 on an on-call basis. Task 3 --100-percent Design Updates Objective: Prepare additional updates to the plans and specifications at the direction of the City,based on 100-percent review comments from the City. Approach: 3.1 Prepare Final Bid Documents:Update plans and specifications based on additional 100-percent review comments from the City,and prepare final bid documents. Provided by the City: a 100-percent design review comments. RH2 Deliverables: e One(1) electronic copy of the,final plans in AuwCAD (dwg) format on a CD. a One (1) electronic copy of the final plans in PDF on a CD, e One(1) electronic copy of the final technical specifications in PDF and Word format on a CD. 0 One(1) full-size hard-copy of final plans. Task 4— Services During Bidding Objective: Provide bidding assistance and engineering services on an on-call basis during the bidding phase for securing a qualified contractor to construct the project. Approach: +1 Respond to Bidder Questions:Respond to bidder questions from the City, and clarify, revise, or change construction plans, technical specifications, or project conditions during the bidding process.
