hereby acknowledge and declare that the before-mentioned company was the prime contractor
for the Agreement known as 2013 Water Treatment Chemicals that was entered into on the
December 21, 2012 (date) between the firm I represent and the City of Kent.
I declare that I complied fully with all of the requirements and obligations as outlined in the City
of Kent Administrative Policy 1.2 and the Declaration City of Kent Equal Employment Opportunity
