Contract Number: ES This is assigned by City Clerk's Office Project Name: 2011 AFSCME Healthcare MOU Description: ❑ Interlocal Agreement ❑ Change Order ❑ Amendment ❑ Contract ® Other: Memorandum of Understanding Contract Effective Date: 11/3/11 Termination Date: Contract Renewal Notice (Days): Number of days required notice for termination or renewal or amendment Contract Manager: Teri Smith Department: Employee Services Detail: (i.e. address, location, parcel number, tax id, etc.): AFSCME Healthcare MOU S•Public\RecordsManagement\Forms\ContractCover\adcc7832 1 11/08 MEMORANDUM OF UNDERSTANDING By And Between THE CITY OF KENT And WASHINGTON STATE COUNCIL OF COUNTY AND CITY EMPLOYEES, AFSCME, AFL-CIO, Local #2617 This Memorandum of Understanding ("MOU") memorializes the agreement between the City of Kent ("City") and the Washington State Council of County and City Employees, AFSCME, AFL-CIO, Local #2617 ("Union"). The City has added 2 additional health care plans that are not identified in the current CBA and have not been available to bargaining unit members. The City and Union agree to provide the following health care plan options to AFSCME employees. I Plans offered effective January 1, 2012, with the November 2011 open enrollment period: A. City's self-insured health insurance program (current 100% plan) administered by Premera Blue Cross and V.S.P Vision; or B. Group Health Cooperative (HMO), includes copays as required by Group Health Cooperative, Premera Blue Cross Dental and V.S.P. Vision plan; or C. Health Savings Account (HSA) and High Deductible Health Plan administered by Premera HSA, with the City funding 35% of the employee's HSA in the first year; or D. 80/20% Traditional Plan administered by Premera Blue Cross PPO. This agreement shall be a one year trial period. Employees may elect any of the above plans and during open enrollment revert back to the 100% Premera plan effective on January 1, 2013. To the extent that this MOU conflicts with any of the terms of the current CBA between the City and the Union, this MOU shall control. This agreement shall not be interpreted to create a past practice nor shall it be interpreted as creating a pattern of action on the part of the City.
