This Change Order amends the above-referenced contract; all other provisions of the contract that are
not inconsistent with this Change Order shall remain in effect For valuable consideration and by mutual
consent of the parties, the project contract is modified as follows:

acknowledges and accepts that this Change Order constitutes final settlement of all claims of any kind or nature
arising from or connected with any work either covered or affected by this Change Order, including, without
limitation, claims related to contract time, contract acceleration, onsite or home office overhead, or lost profits

ATTACHMENT A: SCOPE OF SERVICES
PROJECT STATUS AND UNDERSTANDING
Based upon conversations with the City of Kent Planning Department it has been determined that a
SEPA Checklist and a Shoreline Substantial Use permit is needed for the Lake Meridian Boat
Launch Project Douglass Consulting proposes to complete these 4 tasks
PROPOSED SCOPE OF SERVICES
Task 1: Prepare SEPA Checklist
Task 1 includes conducting research and environmental analysis and preparing a SEPA Checklist for
the Meridian Lake Park Boat Launch project The draft SEPA Checklist is being submitted to the
Client for review and will be submitted to the City of Kent Planning Department with any Client
revisions This task includes a response to comments and a final SEPA Checklist, if needed
This task assumes that the City will make a Determmation of Non-Significance or a Mitigated
Determination of Non-Significance. A finding of Significance would require a higher level
environmental review document which DC can prepare under an Extra Services contract.
Task 2: Prepare Shoreline Substantial Use Permit
This task includes preparing the application forms for the City of Kent Shoreline Substantial Use
permit, as needed, and coordinating with the project team to provide all required attachments
(survey, signatures, etc) for the submittal. The draft Shoreline permit application documents are
being submitted to the Client. Once revisions are incorporated, the permit application will be
submitted to the City of Kent. This task includes responding to requests for additional information
as needed
Task 3: Client and Agency Coordination
Task 3 includes coordination with the Client throughout completion of the project. One meeting at
the City of Kent to complete the SEPA and Shoreline submittal is included in this task. If requested
by the Client, DC will also attend any additional meetings to discuss the Lake Meridian Boat Launch
Project in relation to any identified permit issues
Task 4: Project Management
Task 4 entails administrative tasks such as contracting and invoicing for the project through closure
of the project.
Expenses
Expenses include reproduction of project documents, postage, mileage, and similar project
associated expenses.
FEES AND SCHEDULE
For the preceding Scope of Services, DC proposes to be compensated on a time and expense basis at the
rate of$100 00 per hour The level of effort estimated to complete the tasks is shown as numbers of

hours in the Estimate of Costs table, below. Every effort will be made to complete this work for the
estimated amount listed below No additional work will be conducted under any task item without
Client authorization Monthly invoices will be sent and are due upon receipt.
DC understands project cost and schedule are a concern for the Client. We wish to offer a service that
best suits the Client, therefore, if the task list, project scope or schedule does not meet your needs we
will work with you to establish a proposal that is better tailored to your situation
