("Contractor") CONTRACT NAME &PROJECT NUMBER: West Fenwick Park Design ORIGINAL CONTRACT DATE- December 29, 2005 This Change Order amends the above-referenced contract; all other provisions of the contract that are not inconsistent with this Change Order shall remain in effect. For valuable consideration and by mutual consent of the parties, the project contract is modified as follows. 1. Section I of the Agreement, entitled"Description of Work," is hereby modified as follows: Provide all labor, materials, and equipment necessary to. Provide additional work items to the original scope of work and new work for the ballfield underdrain as described on the Consultant's proposal dated March 31, 2006 attached and incorporated as Exhibit A. 2. I%TLC, )NC 114b NW IEARY WAY SEATTLE, WA 9a107 {d;6)?59 T13i I ax(20e)73�2t3fi March 31,2006 Perry Brooks, Project Manager City of Kent 220—4`h Avenue South Kent, Washington 98032-5895 Re- West Fenwick Park Landscape Architectural Services Amendment#E 1 Dear Perry, Per your request we have prepared the following contract amendment proposal for the West Fenwick Park project Listed below are the additional tasks and associated fees with review continents by the City from the 100% submittal set The tasks and fees are separated into two categories The first category is for revisions and additions to elements contained within the current scope of work The second category is for new work associated with the ballfield underdram system Additional Work Items Scope of additional improvements associated with original scope items to include: 1- New 4' high colored chain link fence along south edge of play equipment area The fence to include an expanded maze similar to sketch piovided by the City Colors to be blue,yellow and red A 4'high galvanized chainlmk fence to continue from the play area east to the path leading to the basketball court Both fences to include a concrete mow strip 2- Revised asphalt path to cement concrete from the parking lot to the new basketball courts 3- Shift ADA stalls to the west and provide a maintenance vehicle access from parking lot 4- Add sidewalk to the east side of the parking lot Connect to the plaza area 5- Revise paving and planting plan for new plaza layout 6- Show location of new waterline, meter and deduct meter to restroom location 7- Revise asphalt basketball court, move bench locations,add grass between walk and court 8- Relocate drinking fountain along path to the new basketball court 9- Add concrete band around outside edge of basketball court Page 2 of 2 Scope of new work associated with ballfield underdratn- I- Review soils report provided by City 2- Determine limit of new underdram system for the ballfield area For this proposal it is assumed that the size of the underdramed field will be approximately 450' X 250' 3- Develop construction drawings showing new underdram system and 9"deep sand/organic/ topsoil mix For this proposal it is assumed that the underdram system outfall will be connected to a sump dram or existing CB and that no downstream design or reports will be required If additional stormwater system design is required, a civil engineer/fee will need to be added to the contract 4- This scope does not include the design of new backstops or infield mixes 5- The construction drawings for the ballfield area will be developed as a separate package for later use by the Ctty. Bidding: Scope and fee to be determined at a later date if requested. Construction Services: Scope and fee to be determined at a later date if requested The following represents our fee proposal for the project Summary of Fees Work Phase Base Fee Additional work items to current scope $2,100 Ballfield Underdram System $4,500 Expenses $300 Total $6,900 All work would be invoiced monthly based upon the percentage of work completed during the billing period. SBA is prepared to continue work immediately,and pioceed through the design phases.
