220 Fourth Ave S Pathogen Support will be an addendum to the medical physical program
Kent,WA ON32-5895 now being conducted through HealthForce

Prevention 253-856-4400 HealthForce will be the provider to handle any and all blood or airborne
exposure incidents experienced by Kent Fire Department employees while
in the line of duty An employee exposed while off-duty will be allowed
access to the same system,but the off-duty employee must pay for the
services in full.
HealthForce will provide a 24/7 (24 hours a day, 7 days a week)
knowledgeable live contact point, who will give the requesting officer a
detailed explanation of the correct procedures to follow for the proper
handling of the exposure
The letter of September 25, 2002 and the Physician Consultation&
Bloodborne Pathogen Support documents are hereby incorporated as
exhibits A and B respectively, as if set forth here in full.
There shall be a one-time set-up fee of S1,500 00 paid to HealthForce from
the City of Kent Fire Department This fee will cover program set-up costs
incurred by HealthForce
All communications on this addendum will be in ng d agreed to by
the City of Kent Fire Department&HealthForce Inc
