and Driver Salesmen and Warehousemen's Union, Local 117,TEAMSTER
The purpose of this memorandum of understanding is to clarify the parties' agreement in regards to
insurance benefits provided Group Health Cooperative.
The parties have met and discussed the changes mandated by Group Health Cooperative to the City
of Kent effective January 1, 1996. The changes have been accepted by both parties. Those changes
include $5.00 copay per office visit and prescription drugs.
To offset the effects of the copay, the parties agree to the following:
* Since Group Health does not offer any vision benefits outside of eye exams, all Teamster
members enrolled in Group Health will have the present PPO Vision Service Plan (VSP) added
to their medical coverage effective January 1, 1996.
* Employee dependent medical premiums in 1996 will remain at the following rates:
Employee Only $0.00
Employee & Spouse $25.00
Employee & Children $15.00
Employee & Family $65.00
The above is the parties' full and complete agreement with regards to Group Health Insurance.
